Documents Needed for NC Medicaid Application for Medicare Savings
The fastest way to stall a North Carolina Medicare Savings Program application is to submit it without the right documents. The federal standard is a determination within 45 calendar days of the application date for non-disability MSP applications. Missing a single verification form adds weeks of back-and-forth, and if requested documents aren't returned in time, the application closes as a procedural denial. Here's exactly what to gather before you file.
The Application Forms
North Carolina requires specific state forms depending on the filing route:
DHB-5200-ia — The standard comprehensive application for health coverage. This is the primary form for MSP applications filed on paper, by mail, or in person at county DSS. It's also the form behind the ePASS online application workflow.
DHB-5202D-ia (Appendix D) — The mandatory income and resources supplement for Aged, Blind, and Disabled coverage. This is the form that most incomplete applications are missing. Without Appendix D, DSS cannot evaluate financial eligibility for MQB-Q, MQB-B, or MQB-E. An application submitted without it will trigger a request for the supplement, adding weeks to the processing timeline or resulting in a procedural denial.
DHB-5202C-ia (Appendix C) — The Authorized Representative designation form. If an adult child is handling the application for a parent, this form gives them legal authority to submit documents, receive notices, and communicate with county DSS on the parent's behalf. Without it, DSS staff cannot discuss case details with you, regardless of your relationship to the applicant.
Income Verification Documents
County caseworkers need proof of every income source for the applicant (and spouse, if married and living together):
- Social Security benefit verification letter — The current SSA letter shows the gross monthly payment. You can print it from ssa.gov or request it by calling 1-800-772-1213.
- Pension statements — Monthly or annual statements from any private pension, annuity, or retirement plan disbursements. VA benefits statements if applicable (Aid and Attendance or housebound allowances are excluded from income, but the base VA pension counts).
- Earned income documentation — If either spouse works, provide recent pay stubs or a signed statement from the employer.
- Interest and dividend statements — Bank statements or 1099-INT/1099-DIV forms showing interest earned on savings accounts, CDs, or investment accounts.
North Carolina applies the $20 general unearned income disregard before comparing against MQB thresholds. The caseworker deducts this from gross unearned income, so your parent's gross Social Security check can be up to $20 above the published limit and still qualify.
Resource Verification Documents
For April 1, 2026 through March 31, 2027, the MSP resource limits are $9,950 (individual) and $14,910 (couple). DSS needs documentation for every countable asset:
- Bank statements — Checking, savings, money market, and CD accounts. Provide the most recent full monthly statement for each account. North Carolina's Asset Verification System (AVS) cross-references financial institutions electronically, so undisclosed accounts will surface during processing.
- Investment account statements — Brokerage, mutual fund, stock, and bond accounts showing current market values.
- Retirement accounts — IRA, 401(k), and Keogh account statements. North Carolina counts these as resources to the extent the funds can be withdrawn, minus any early withdrawal penalties.
- Life insurance policies — Face value documentation for all policies. Policies with a combined face value of $1,500 or less per person are exempt. Above that threshold, the cash surrender value becomes a countable resource.
- Vehicle registration — One primary vehicle per household is exempt regardless of value. Additional vehicles are counted at fair market value.
- Burial arrangements — Documentation of prepaid irrevocable burial contracts or designated burial funds (exempt up to $1,500 per individual, reduced by any excluded life insurance face value).

What's Exempt (Don't Over-Document)
These resources are excluded from the calculation, so while you should be prepared to confirm them, they won't count against the limit:
- Primary residence (including surrounding land) — no home equity cap for MSP, unlike long-term care Medicaid
- One vehicle per household
- Household goods, furniture, personal effects
- Burial spaces and plots for the applicant, spouse, and immediate family
Identity and Residency
- Photo ID — Driver's license, state ID, or passport
- Medicare card — Showing the Medicare Beneficiary Identifier (MBI) and current enrollment in Part A and/or Part B. If applying for QMB while seeking Part A buy-in, include the SSA conditional Part A documentation.
- Proof of North Carolina residency — Utility bill, lease agreement, or mail at the current address. The applicant must demonstrate physical presence with intent to remain permanently or indefinitely.
- Citizenship or immigration status — Birth certificate, U.S. passport, or qualifying immigration documentation. Lawful Permanent Residents must meet the five-year residency requirement where applicable.
Filing Routes
Once documents are assembled, the application can go through any of these channels:
- ePASS portal (epass.nc.gov) — Create an NCID account, select Medical Assistance, and indicate Medicare enrollment to route to the ABD queue
- In person at the county DSS office
- By mail to the county DSS
- By phone — Local DSS caseworkers can take telephone applications
- By fax or secure email to county DSS
One important note: talking to a DSS receptionist or community group volunteer does not establish a protective filing date. The application must be formally signed and submitted.
