Vermont Part A Buy-In for QMB: How It Works When Your Parent Lacks Free Part A
Your parent didn't accumulate 40 quarters of Medicare-covered employment. They're facing a Part A premium of $311 or $565 per month — on top of the $202.90 Part B premium — and they can't afford either. Vermont is a Part A Buy-In State, which means the state will pay that Part A premium for QMB-eligible residents. But getting there requires navigating a process that trips up applicants and even SSA field office staff.
What Part A Buy-In Means
Most people get Medicare Part A premium-free because they (or a spouse) paid Medicare taxes for at least 40 quarters. People with fewer than 40 quarters must pay a monthly Part A premium: $311 with 30–39 quarters, $565 with fewer than 30 quarters, in 2026.
Vermont maintains a Part A state buy-in agreement with CMS. When a resident is approved for QMB, the state directly absorbs the Part A premium through the buy-in system — the beneficiary does not receive a Part A premium bill. The state also picks up Part B ($202.90/month), all deductibles, and all coinsurance. For someone without free Part A, QMB can be worth over $10,000 per year.
The Conditional Part A Problem
QMB requires Part A entitlement, but Vermont's route depends on whether your parent already has Part B. Under SSA POMS HI 00801.140, someone who already has Part B and lacks premium-free Part A applies to the state for QMB; if approved, Vermont can add Part A through its buy-in agreement. If your parent does not yet have Part B, SSA's conditional enrollment process can establish Part A without starting Part A premiums before QMB approval. Use the sequence below for that situation:
- If your parent does not yet have Part B, they visit a local Social Security field office and file a Conditional Part A application, explicitly stating enrollment is conditional on state QMB approval
- SSA records conditional Part A enrollment so Part A coverage begins only if the state approves QMB; your parent remains responsible for Part B premiums until QMB begins
- Your parent submits Form 201P to Green Mountain Care with proof of the conditional Part A filing attached
- DVHA approves QMB and transmits a Part A buy-in accretion record to CMS, taking over the premium permanently
The Circular Referral Loop
In practice, the process can break down when an SSA field office or state eligibility specialist uses the wrong route. For applicants already enrolled in Part B, SSA directs the field office to refer them to Vermont to apply for QMB; if approved, Vermont can add Part A through its buy-in agreement. Applicants who do not yet have Part B may need the conditional Part A process, and confusion there can create a referral loop.
If your parent already has Part B, apply to Green Mountain Care using Form 201P and ask DVHA to coordinate the state Part A buy-in. If your parent does not yet have Part B, explicitly request a "Conditional Part A Application under POMS HI 00801.140" at the SSA field office. Get a copy of the SSA transaction receipt showing the conditional enrollment was established, then upload it with Form 201P.
If the SSA representative is unfamiliar with the process, ask to speak with a claims specialist or technical expert at the office. The POMS reference (HI 00801.140) is the internal manual citation they can look up.

Enrollment Timing
Part A enrollment through Vermont's buy-in is not restricted to the General Enrollment Period (January–March). A QMB-eligible person who already has Part B can be added to Part A through the state buy-in. If they do not yet have Part B, SSA's conditional Part A process can be used any time of year in a Part A Buy-In State. This is a significant advantage — someone who discovers QMB eligibility in August doesn't have to wait until January to start the process.
