West Virginia Part A Buy-In Through QMB: Getting Medicare Without Paying the Premium

The Part A Premium Problem

Most people get Medicare Part A for free because they or a spouse paid Medicare taxes for at least 40 quarters (10 years). But if you don't have enough work credits, Part A isn't free — in 2026, the full premium is $565/month for people with fewer than 30 quarters; those with 30 to 39 quarters pay the reduced premium of $311/month.

That's a $6,780 annual expense before you even use any medical services. For someone who qualifies for QMB based on their income, paying $565/month out of pocket to get Part A defeats the purpose of the program.

West Virginia is a Part A Buy-In state. That means the state will pay your Part A premium through QMB, and you can use a conditional enrollment process to avoid paying anything upfront.

How Conditional Part A Enrollment Works

The process involves two agencies — SSA and DoHS — working in sequence:

Step 1: Visit your local Social Security office. Request a conditional Part A application under SSA POMS HI 00801.140. Tell the representative explicitly: "My application for Medicare Part A is conditional upon the state approving my application for QMB coverage."

SSA creates a conditional Part A enrollment — meaning Part A activates only if the state approves QMB. You don't pay any premiums during this conditional period.

Step 2: Apply for QMB with West Virginia DoHS. Take the SSA conditional receipt and submit Form DFA-QSQ-1 or an application through WV PATH to the Bureau for Family Assistance.

Step 3: State approval triggers the Buy-In. When DoHS approves QMB, the Bureau for Medical Services sends a transaction code through the CMS Buy-In exchange. CMS bills West Virginia for your Part A premium going forward. Part A activates without you owing anything.

If SSA Pushes Back

Some SSA field office representatives aren't familiar with conditional Part A applications and may insist you pay the premium upfront before they'll process enrollment. This is incorrect.

If you encounter resistance, ask for a supervisor and cite SSA POMS HI 00801.140 — the internal operating instruction that authorizes conditional enrollment. You can also contact WV SHIP (1-877-987-4463) for help navigating the SSA interaction. SHIP counselors are trained on this process and can advocate on your behalf.

Timing Considerations

QMB coverage starts the first day of the month after approval — it cannot be backdated. The conditional Part A enrollment tracks the same timeline. Once both sides approve, Part A coverage begins prospectively.

The 60-to-90-day interagency processing window applies here too. During that period, the Buy-In data exchange between DoHS, CMS, and SSA is still syncing. As long as you have the DoHS approval notice confirming QMB, you have coverage — even if the systems haven't fully caught up.

Who This Applies To

Conditional Part A enrollment through QMB is relevant for people who:

  • Didn't work enough quarters in covered employment to qualify for premium-free Part A
  • Lost premium-free Part A after returning to work above the substantial gainful activity level (in which case QDWI, a separate program with higher income limits, may also apply)
  • Immigrated to the US later in life and don't have a US work history
  • Worked primarily in jobs not covered by Social Security (some state and local government positions)

If you already have premium-free Part A, QMB still covers your Part B premium and all Medicare cost-sharing. The Buy-In process only matters when you'd otherwise have to pay for Part A.

The West Virginia Medicare Savings Programs guide walks through the conditional Part A application step by step, including what to say at the SSA office and how to follow up if the Buy-In data exchange stalls.
