SHINE Program Massachusetts: Free Medicare Counseling for Seniors

Your parent just turned 65, the Medicare paperwork is piling up, and every website you land on wants to sell you an insurance plan. Massachusetts runs a program that cuts through that noise entirely — and it costs nothing.

What SHINE Actually Does

SHINE stands for Serving the Health Insurance Needs of Everyone. It is Massachusetts's version of the federal State Health Insurance Assistance Program (SHIP), operated by the Executive Office of Aging & Independence. Certified SHINE counselors provide free, unbiased, one-on-one help with Medicare decisions — no insurance sales, no commissions.

A SHINE counselor can help with:

  • Comparing Original Medicare against Medicare Advantage plans
  • Reviewing Part D prescription drug plan options against your parent's actual medications
  • Screening for Medicare Savings Programs (QMB, SLMB, QI) that pay the $202.90 monthly Part B premium
  • Walking through Prescription Advantage, the state's own pharmacy assistance program covering incomes up to 500% of the federal poverty level
  • Translating confusing MassHealth notices and determination letters
  • Filing MSP applications and Extra Help (Part D Low-Income Subsidy) paperwork

SHINE counselors do not make eligibility determinations — that is MassHealth's role. What they do is prepare your application, screen for every program you might qualify for, and make sure nothing falls through the cracks.

Where to Find a SHINE Counselor

SHINE counselors work out of local Councils on Aging (COAs), Aging Services Access Points (ASAPs), community health centers, and senior centers across the state. The fastest way to connect:

  • MassOptions helpline: 1-800-243-4636 (TTY: 1-800-439-2370). Tell them you need a SHINE appointment and they will route you to the nearest counselor.
  • Online: Visit the SHINE program page at mass.gov/shine-program for counselor locations by town.
  • Your local Council on Aging: Most towns list SHINE hours on their senior center website or monthly newsletter.

Appointments are available in person, by phone, and in some locations by video. SHINE also offers free interpreter services for non-English speakers.

The Booking Problem — and How to Work Around It

SHINE is heavily used, and during peak periods the wait can stretch to weeks. The annual Medicare Open Enrollment window (October 15 through December 7) and the weeks after March 1 — when new federal poverty level figures take effect and MassHealth income limits update — are the hardest times to get an appointment.

If your parent received a MassHealth redetermination notice with a 30-day response deadline, waiting for a SHINE appointment may not be realistic. Three things that help:

  1. Call early in the enrollment season. September appointments for October enrollment fill first — book in late August if possible.
  2. Try smaller towns. Rural COAs in western Massachusetts or on the Cape sometimes have shorter wait lists than Greater Boston locations.
  3. Prepare before the appointment. The more organized your paperwork is, the more a counselor can accomplish in a single session. Bring your parent's Medicare card, Social Security benefit statement, any pension or income documentation, a list of current prescriptions, and the MassHealth notice that triggered the appointment.

What SHINE Cannot Do

SHINE counselors are not attorneys and do not represent you at MassHealth Board of Hearings proceedings. If your parent was denied MSP benefits and you need to file a formal appeal, SHINE can explain the process and help gather documentation, but you may need legal assistance for the hearing itself. Greater Boston Legal Services and Community Legal Aid both handle MassHealth appeals for low-income seniors at no cost.

SHINE also does not replace a financial planner or elder law attorney for complex situations like long-term care Medicaid spend-downs or irrevocable trust planning. Their lane is Medicare and related cost-help programs.

Getting Started

If your parent is paying the full $202.90 monthly Part B premium out of their Social Security check and their income is under $2,993 per month, there is a strong chance they qualify for Massachusetts's Medicare Savings Programs — which would eliminate that premium entirely. A SHINE counselor can run the numbers in a single appointment.
