RIPAE: Rhode Island Pharmaceutical Assistance to the Elderly Program Guide

What RIPAE Covers

RIPAE (Rhode Island Pharmaceutical Assistance to the Elderly) is a state-funded program that helps pay for generic prescription drugs during the Medicare Part D annual deductible stage. In 2026, a Part D plan can have a deductible of up to $615. RIPAE reimburses a percentage of qualifying generic drug costs paid during the plan's deductible stage.

RIPAE does not cover brand-name medications, Part D premiums, or drug costs incurred after you exit the deductible stage and enter the Initial Coverage phase. It's narrowly focused on the deductible gap for generics.

The program is administered by the Rhode Island Office of Healthy Aging (OHA), not DHS. It's a completely separate application from the Medicare Savings Programs.

2026 Income Tiers and Benefits

RIPAE has no asset or resource limit — eligibility is based entirely on gross annual income. The tiers are adjusted each January using the Social Security COLA.

Level Who Qualifies Single Income Couple Income State Pays You Pay
Level 1 Seniors 65+ Under $39,002 Under $50,319 60% 40%
Level 2 Seniors 65+ $39,002–$48,965 $50,319–$61,227 30% 70%
Level 3 Seniors 65+ $48,966–$85,848 $61,228–$85,848 15% 85%
Level 4 Disabled 55–64 (SSDI) Up to $85,848 Up to $85,848 15% 85%

Level 1 also includes free Rhode Island State Beach passes. Levels 2, 3, and 4 grant a Special Enrollment Period (SEP) that lets you switch your Part D or Medicare Advantage plan once per year outside of the regular Open Enrollment window (October 15–December 7), though the SEP can't be used during that window.

How Reimbursement Actually Works

RIPAE does not reduce your copay at the pharmacy counter. You pay the full cost of your generic prescriptions during the deductible stage, save your pharmacy receipts, and submit them to OHA. The state reimburses its share quarterly by check.

This means you need upfront cash for the drugs and the discipline to keep receipts organized. The quarterly cycle also means reimbursement arrives weeks to months after you paid — not ideal for someone on a tight fixed income, but it's real money back.

Who Can't Enroll

Two groups are excluded under RIPAE eligibility rules:

Medicaid recipients. If you're on full Rhode Island Medicaid, you already have comprehensive drug coverage. RIPAE is designed for people who fall between the Medicaid and Extra Help safety nets.

Extra Help (LIS) recipients. Anyone receiving full Extra Help through Medicare — including everyone enrolled in QMB or QI — is ineligible for RIPAE. Extra Help already sets generic copays at $5.10 and eliminates the Part D deductible entirely. RIPAE would be redundant.

This creates an important decision point for people whose income is above MSP limits but below $39,002. If you don't qualify for QMB or QI (and therefore don't get automatic Extra Help), RIPAE Level 1 is the next best option for drug cost relief.

The Outdated "Donut Hole" Language

OHA's printed brochures still describe RIPAE as covering drugs in the Part D "donut hole." That's outdated. The Inflation Reduction Act eliminated the coverage gap entirely. The 2026 Part D benefit has three stages: deductible (up to $615), initial coverage (standard copays until $2,100 out-of-pocket), and catastrophic ($0 copays after $2,100). RIPAE operates in the deductible stage only.

If you're comparing RIPAE materials to current Part D plan documents and the terms don't match, it's because the state brochures haven't been updated to reflect the redesign.
