Rhode Island Extra Help for Medicare Part D: Eligibility and How to Apply

What Extra Help Covers

Extra Help — officially the Medicare Part D Low-Income Subsidy (LIS) — is a federal program that dramatically cuts prescription drug costs for low-income Medicare beneficiaries. Full Extra Help in 2026 provides:

  • $0 Part D premium when enrolled in a benchmark Part D plan
  • $0 annual deductible (vs. the standard $615 maximum)
  • $5.10 copay for generic and preferred multi-source drugs
  • $12.65 copay for brand-name drugs
  • $0 copay once you reach the $2,100 annual out-of-pocket cap (catastrophic coverage)

Compared to standard Part D, where you pay your plan's deductible and then plan-specific cost-sharing until hitting the cap, Extra Help saves hundreds to thousands of dollars per year depending on your medications.

Automatic Enrollment Through Rhode Island's MSP

If you're approved for QMB or QI through Rhode Island DHS, you're automatically deemed eligible for full Extra Help. You don't need to file a separate application with Social Security — the enrollment happens through a data exchange between DHS and CMS.

This is one of the most valuable aspects of qualifying for a Medicare Savings Program. Even QI, which only pays the Part B premium, triggers full Extra Help with its $0 deductible and low copays.

Applying Separately Through SSA

If you don't qualify for QMB or QI but still have limited income and assets, you can apply for Extra Help directly through the Social Security Administration. The income and asset limits for Extra Help are different from the MSP limits and tend to be more generous.

Apply online at ssa.gov, by calling 1-800-772-1213, or at your local SSA field office. SSA's application is Form SSA-1020.

There's an important link between the two programs: when you apply for Extra Help through SSA, federal law (MIPPA) requires SSA to transmit your data to Rhode Island DHS. DHS then treats that transmission as an initiated MSP application. DHS will mail you a supplemental document request — usually asking for asset verification — to complete the MSP evaluation. If you return those documents within the deadline (typically 30 days), you may end up qualifying for QMB or QI as well, getting even more coverage.

Extra Help vs. RIPAE

Extra Help and RIPAE (Rhode Island Pharmaceutical Assistance to the Elderly) are mutually exclusive. If you receive full Extra Help, you cannot enroll in RIPAE — and you wouldn't want to, since Extra Help is far more comprehensive.

RIPAE reimburses a percentage of generic drug costs during the Part D deductible stage only. Extra Help eliminates the deductible entirely and caps copays at $5.10/$12.65 across all coverage stages. The only scenario where RIPAE is the better program is when your income exceeds both MSP and Extra Help limits but falls under RIPAE's thresholds (up to $85,848/year for Level 3).

The 2026 Part D Out-of-Pocket Cap

The Inflation Reduction Act introduced a hard $2,100 annual out-of-pocket cap on Part D drug costs. Once your true out-of-pocket spending hits $2,100, all covered Part D drugs cost $0 for the rest of the year.

For Extra Help recipients, this cap is largely academic — your maximum exposure under full LIS is already minimal. But for people who don't qualify for Extra Help or RIPAE, the $2,100 cap is a meaningful backstop that didn't exist before 2025.

The Rhode Island Medicare Savings Programs, Extra Help & RIPAE Guide explains how to determine which combination of programs — MSP, Extra Help, and RIPAE — gives you the maximum prescription and premium savings based on your income level.
