Rhode Island QMB and SLMB Merged in 2026: What Changed for Beneficiaries

What Changed on February 1, 2026

Rhode Island EOHHS eliminated the Specified Low-Income Medicare Beneficiary (SLMB) tier as a separate program category. Every beneficiary who previously qualified as SLMB is now enrolled in QMB — the more comprehensive Medicare Savings Program tier.

The old structure had three tiers: QMB (income up to 100% FPL), SLMB (100%–120% FPL), and QI (120%–135% FPL). SLMB only covered the Part B premium. Starting February 2026, the QMB income ceiling absorbed the SLMB range, so anyone with income up to 120% FPL now gets full QMB benefits.

Rhode Island still lists SLMB in its federal Medicaid State Plan — CMS requires it — but operationally, every SLMB-eligible applicant is upgraded to QMB automatically.

What Former SLMB Recipients Gained

The difference between SLMB and QMB was substantial:

Benefit SLMB (old) QMB (new)
Part B premium ($202.90/mo) Covered Covered
Part B deductible ($283/yr) Not covered Covered
Part B coinsurance (20%) Not covered Covered
Part A hospital deductible ($1,736) Not covered Covered
Part A coinsurance (days 61+) Not covered Covered
SNF coinsurance (days 21–100) Not covered Covered
Balance billing protections No Yes — Medicare-enrolled providers cannot bill you for Medicare cost-sharing

The balance billing protection alone is a major upgrade. Under SLMB, a hospital could bill you the $1,736 Part A deductible after a hospitalization. Under QMB, that bill is prohibited by federal law.

Checking Your Status

If you were enrolled in SLMB before February 2026, DHS should have transitioned you automatically. But automated transitions don't always execute cleanly. Here's how to verify:

Check your DHS notice. Look for the program category shown on a recent notice. If it doesn't show QMB, call DHS at 1-855-697-4347 and ask which program category your case shows.

If a provider bills you. Keep your DHS notice confirming QMB status and show it to the billing office. Medicare-enrolled providers cannot bill you for Medicare cost-sharing during eligible QMB months.

If you are unsure which program category your case shows, call DHS at 1-855-697-4347 to confirm.

Who Benefits From the Expansion

This mainly affects people who previously qualified only for SLMB and now fall within Rhode Island's QMB income limits.

Previously, these people only got their Part B premium paid. Now they get full protection against Medicare's cost-sharing charges — hospital deductibles, outpatient coinsurance, and the legal shield against balance billing.

For anyone previously denied QMB because their income was slightly over the old 100% FPL cutoff but below 120%, it's worth reapplying. The expanded QMB limit of $1,683/month may now cover you.
