Rhode Island Medicare Advantage vs QMB: How They Work Together
A common question from families in Rhode Island: if a parent has a Medicare Advantage plan and qualifies for QMB, do they need to drop the MA plan? The answer is no — QMB can work alongside Medicare Advantage and protects eligible enrollees from Medicare cost-sharing for covered services.
QMB Protections Apply to Medicare Advantage
Federal QMB balance billing protections cover services under both Original Medicare and Medicare Advantage. Under 42 CFR § 447.15, no Medicare-enrolled provider can bill a QMB enrollee for Medicare cost-sharing — regardless of which Medicare plan the beneficiary has.
That means a parent with a Medicare Advantage plan who gets QMB will not owe:
- The MA plan's in-network copays for doctor visits, specialist visits, and hospital stays
- The Part B premium ($202.90/month, which QMB pays)
- Any Part A or Part B deductibles
The MA plan's copayment schedule still technically exists, but the provider cannot collect it from the QMB enrollee. The provider either bills Rhode Island Medicaid for the cost-sharing amount or writes it off.
The Practical Reality in Rhode Island
Rhode Island Medicaid uses a "lesser-of" reimbursement formula. When a Medicare Advantage plan pays a provider more than the Rhode Island Medicaid fee schedule would allow, Medicaid pays the provider $0 for the cost-sharing. The provider must write off the balance — they cannot bill the QMB enrollee for it.
This creates friction with some providers. A small number of practices may not understand the billing rules and send a QMB enrollee a bill for a copay anyway. That bill is illegal. If it happens, contact the provider's billing office with your DHS QMB approval notice and cite the federal balance billing prohibition. If the provider refuses to adjust, call 1-800-MEDICARE.
Should You Switch to Original Medicare?
There is no requirement to switch. But some families find that Original Medicare plus QMB is simpler. With Original Medicare, all cost-sharing goes through the standard Medicare billing system, and QMB covers it automatically. There are no network restrictions on providers.
With Medicare Advantage, you stay within the plan's network rules, but QMB still protects you from cost-sharing within that network. If your parent is happy with their MA plan's network and benefits (dental, vision, hearing aids), keeping it makes sense.
QMB approval also triggers automatic Extra Help, which reduces Part D drug costs regardless of whether the drug coverage comes through a standalone Part D plan or a Medicare Advantage plan with prescription coverage.
