Arizona QMB With Medicare Advantage or Medigap: What You Need to Know

You're enrolling a parent in Arizona's QMB program, and they already have a Medicare Advantage plan. Or maybe they have Original Medicare with a Medigap supplement. Either way, you need to know: does QMB work alongside these plans, or does something have to go?

QMB and Medicare Advantage: They Work Together

Yes, you can have QMB and a Medicare Advantage plan in Arizona at the same time. QMB pays the Part B premium ($202.90/month in 2026) regardless of whether the beneficiary is on Original Medicare or a Medicare Advantage plan.

For cost-sharing, the interaction depends on the MA plan type:

With a standard Medicare Advantage plan: QMB protection also applies to Medicare Advantage plan cost-sharing. Providers cannot bill QMB enrollees for plan copays or deductibles.

With a Dual-Eligible Special Needs Plan (D-SNP): D-SNPs are designed for people who have both Medicare and Medicaid, including MSP-only Medicaid. They coordinate benefits between the programs. Eligibility and cost-sharing depend on the specific plan and the beneficiary's Medicaid status. Ask Arizona SHIP about current options in Maricopa and Pima counties.

One important point: QMB approval does not force a beneficiary off their existing Medicare Advantage plan or onto a specific plan. They can keep whatever MA plan they're enrolled in. But during the Annual Election Period (October 15 through December 7), it's worth reviewing whether a D-SNP would provide better cost coordination.

QMB and Medigap: Usually Unnecessary

Medigap (Medicare Supplement) policies exist to cover the gaps in Original Medicare — the deductibles, coinsurance, and copayments that Part A and Part B don't pay. QMB covers those same costs. So if your parent has QMB, a Medigap policy is largely redundant.

Here's the practical scenario: a senior bought a Medigap Plan F years ago when they were paying everything out of pocket. Now they qualify for QMB. The Medigap policy is still technically valid, but it has almost nothing left to cover because QMB is picking up the cost-sharing. They're paying a monthly Medigap premium for coverage that QMB provides at no cost.

Should they drop the Medigap policy? Before dropping it, ask Arizona SHIP what happens if QMB ends and whether your parent could get Medigap again without medical underwriting. Outside guaranteed-issue windows, insurers may use medical underwriting to deny coverage or charge higher premiums.

Before making a decision, contact Arizona SHIP for free, unbiased plan counseling and confirm the consequences of dropping the policy.

The Balance-Billing Rule Applies Either Way

Regardless of whether a QMB beneficiary uses Original Medicare, a Medicare Advantage plan, or has a Medigap policy, the federal balance-billing prohibition under Section 1902(n)(3) of the Social Security Act still applies. Providers cannot bill a QMB patient for Medicare deductibles, coinsurance, or copayments. Period.

If a provider sends a bill that should be covered by QMB protections, the beneficiary should not pay it. Our post on Arizona QMB balance billing protections covers how to dispute those charges.

Choosing the Right Setup

The decision tree is simpler than it looks:

  • QMB + Original Medicare: QMB pays Part B premium and all cost-sharing. No need for Medigap.
  • QMB + Medicare Advantage (standard): QMB pays the Part B premium and protects against billing for plan copays and deductibles.
  • QMB + D-SNP: Ask Arizona SHIP whether the beneficiary is eligible and whether a plan's network and costs fit their needs.

The Arizona Medicare Savings Programs & Extra Help Guide walks through each scenario with specific steps for evaluating plan options after QMB approval — including when to contact Arizona SHIP (the State Health Insurance Assistance Program) for free, unbiased plan counseling.
