Arizona Medicare and AHCCCS Dual Eligible: MSP Benefits and Extra Help
A full-benefit dual eligible in Arizona has both Medicare and full AHCCCS Medicaid. Medicare Savings Program enrollment can also help people who aren't enrolled in full Medicaid. For the roughly 210,000 to 230,000 Arizonans in a Medicare Savings Program, that enrollment unlocks prescription drug help that many don't realize comes automatically.
How MSP Enrollment Triggers Extra Help
When AHCCCS approves someone for QMB, SLMB, or QI, it transmits daily enrollment files to CMS. CMS updates the federal Medicare database and assigns "Full LIS" (Low-Income Subsidy) status — commonly called Extra Help — without the beneficiary filing a separate application at Social Security.
This automatic deeming is the key advantage of applying through AHCCCS rather than going directly to SSA for Extra Help. A standalone Extra Help application at Social Security still enforces a federal resource limit ($17,600 individual / $35,130 couple). Arizona's MSP application has no asset test. So a person with $25,000 in savings who would be denied standalone Extra Help by SSA can get it through the AHCCCS route by securing MSP approval first.
What Full Extra Help Provides in 2026
The prescription drug savings stack on top of the Part B premium assistance:
- Part D monthly premium: Fully covered up to the Arizona benchmark plan amount — no out-of-pocket Part D premium
- Annual Part D deductible: Eliminated ($0)
- Prescription copays: Capped at $5.10 for generic drugs and $12.65 for brand-name drugs
- Catastrophic coverage: After reaching the $2,100 annual out-of-pocket cap (new under the Inflation Reduction Act), copays drop to $0
- Special Enrollment Period: Extra Help beneficiaries can switch Part D plans once per quarter during the first three quarters of the year, outside the standard Annual Enrollment Period
If a newly approved MSP beneficiary visits a pharmacy and the system doesn't show LIS copay rates, CMS requires pharmacies to apply "Best Available Evidence" — a copy of the AHCCCS/DES QMB approval notice or an active AHCCCS ID card is enough to manually trigger the reduced copays at the counter.
Auto-Assignment to a Part D Plan
If someone receiving Full LIS is not enrolled in a standalone Part D plan, CMS auto-assigns them to a zero-premium benchmark plan in Arizona. The beneficiary doesn't need to do anything — drug coverage is activated automatically.
The auto-assigned plan may not be the best fit for a specific medication list. Extra Help recipients can switch plans quarterly, so reviewing the formulary and switching to a plan that covers needed medications at preferred pharmacies is worth doing once the initial coverage is in place.

QMB Plus: Full Dual Eligibility
Some Arizonans meet the criteria for both QMB and full AHCCCS Medical Assistance (or ALTCS). These "QMB Plus" beneficiaries receive the complete package: Medicare premium and cost-sharing coverage through QMB, plus full Medicaid benefits through AHCCCS for services Medicare doesn't cover — dental, vision, long-term care services, and more.
QMB Plus isn't a separate application. If someone qualifies for both QMB based on income and full AHCCCS based on their eligibility category (SSI recipients, for example), the system assigns both.
The Bottom Line on Dual Benefits
For a senior paying $202.90/month for Part B plus $100–$200/month in prescription copays, the combined MSP and Extra Help savings reach $4,000–$5,000 per year. All from a single AHCCCS application.
