AHCCCS Income Limits 2026: Medicare Savings Program Eligibility
Your parent's Social Security check lands every month already short — $202.90 gone to the Part B premium before they buy groceries. If their income falls under Arizona's 2026 AHCCCS limits for Medicare Savings Programs, the state pays that premium instead.
2026 Income Limits for Arizona Medicare Savings Programs
AHCCCS runs three Medicare Savings Programs, each tied to a percentage of the Federal Poverty Level. The 2026 guidelines took effect for AHCCCS MSP determinations on February 1, 2026; these are the numbers in effect for 2026:
| Program | FPL Band | Individual Limit | Couple Limit | What It Covers |
|---|---|---|---|---|
| QMB | ≤ 100% FPL | $1,350/month | $1,824/month | Part A premium, Part B premium, all deductibles and copays |
| SLMB | >100–120% FPL | $1,616/month | $2,184/month | Part B premium only |
| QI | >120–135% FPL | $1,816/month | $2,455/month | Part B premium only (capped federal funding) |
Those limits include the $20 unearned income disregard that AHCCCS applies automatically. The published AHCCCS manual shows $1,330 for QMB — the effective limit is $1,350 because the first $20 of unearned income doesn't count.
How AHCCCS Counts Income
AHCCCS uses gross Social Security before any deductions — before the Part B premium, before tax withholding, before voluntary garnishments. That's the number on the SSA benefit verification letter, not the amount deposited in the bank account.
For people who still work, Arizona applies standard SSI earned-income exclusions: the first $65 of monthly earnings is excluded, then half of what remains. Private pensions and annuity distributions count in full. VA disability compensation counts, but Aid and Attendance and housebound allowances are excluded.
If an adult child pays a parent's utility bill or rent directly to the vendor, that payment is excluded under AHCCCS in-kind support rules. Cash given directly to the parent counts as unearned income.
Arizona's No-Asset-Test Rule
Arizona eliminated the asset test for QMB, SLMB, and QI. Bank accounts, retirement funds, home equity, vehicles — none of it matters for Medicare Savings Program eligibility. The federal baseline asset limits ($9,950 individual / $14,910 couple) that Medicare.gov and national guides publish do not apply in Arizona.
This is different from ALTCS (Arizona Long Term Care System), which enforces a $2,000 individual asset limit. Families confuse the two programs constantly — ALTCS covers nursing-home-level care, while Medicare Savings Programs cover Medicare premiums and cost-sharing for people living in the community.

What Happens When Income Changes
Social Security cost-of-living adjustments take effect each January. AHCCCS holds a January disregard: new FPL guidelines don't take effect until February 1, so a COLA bump won't knock anyone off coverage during January.
If the Social Security Fairness Act (WEP/GPO repeal) increased a parent's monthly benefit, that higher ongoing amount does affect eligibility. The retroactive lump sum is excluded as both income and resources under federal Medicaid accounting rules. The ongoing increase isn't — report it to AHCCCS within 10 calendar days.
When income rises above one MSP tier but stays below the next, AHCCCS shifts coverage. A parent who crosses the QMB threshold might qualify for SLMB instead, still getting the Part B premium paid.
Next Steps
Check your parent's gross Social Security amount against the table above. If it falls below $1,816 for an individual (or $2,455 for a couple), there's a program that covers at least the Part B premium.
