North Dakota Rural Seniors: Medicare Savings Help in Frontier Counties
The Distance Problem
North Dakota has 36 rural and frontier counties where the nearest Human Service Zone office might be a 90-minute drive — longer in winter, when roads close or a senior can't safely make the trip. Counties like Slope, Divide, and Towner have some of the lowest population densities in the country, and the consolidation of the old county social service offices into 19 regional Human Service Zones made the distances even greater for some communities.
The Medicare Savings Programs (QMB, SLMB, QI) don't require in-person visits. Every step of the application process can be done remotely.
Three Ways to Apply Without Driving
Phone: Call the ND HHS Customer Support Center at 1-866-614-6005 (TTY: 711). A representative walks through the application with you and tells you which documents to mail or fax. This is the most accessible option for seniors who aren't comfortable with computers.
Online portal: The ND HHS Self-Service Portal at hhs.nd.gov accepts applications 24/7. You create an account, fill in the health care application, upload scanned or photographed documents, and track the case status. An adult child in another city can do this on the parent's behalf with an Authorized Representative designation.
Mail: Download Form SFN 958 from the ND HHS website, fill it out, and mail it with copies of supporting documents to the ND HHS Customer Support Center, PO Box 5562, Bismarck, ND 58506. Don't send originals — the state needs copies only.
All three channels feed into the same centralized processing system. There is no advantage to walking into an office.
Rural Asset Rules That Help
North Dakota's MSP asset exclusions are especially relevant in rural and farming communities:
- Homestead land: Rural homesteads include up to 160 contiguous acres; urban homesteads include up to two acres. The primary residence is fully excluded from countable resources regardless of value.
- Agricultural property: Machinery, farm equipment, livestock, and harvested produce retained for seed or feed are excluded as property essential to earning a livelihood, provided the applicant is actively engaged in agricultural operations.
- CRP land: Land enrolled in the federal Conservation Reserve Program is excluded under the same essential-property provision.
- Mineral rights: Non-producing mineral acres are excluded. Producing mineral interests are also excluded if the owner demonstrates a good-faith effort to sell by offering the rights to at least three local buyers or through public advertising. Royalties from producing mineral leases count as unearned income in the month received.
- One vehicle: Fully excluded regardless of market value, as long as it's used for transportation.
These exclusions mean a ranching family that owns land, equipment, and mineral rights can still qualify for QMB if their monthly income and liquid savings fall within the thresholds.

SHIC Outreach for Rural Counties
The State Health Insurance Counseling (SHIC) program, run by the North Dakota Insurance Department, provides free Medicare counseling over the phone at 1-888-575-6611. SHIC counselors can explain MSP rules, help estimate eligibility, and walk through Part D plan options — all by phone. ND HHS makes the eligibility determination. SHIC counselors can't fill out the application for you or serve as your authorized representative, but they can explain which forms to file and what documents to gather.
SHIC also operates regional outreach events in smaller communities. Check the Insurance Department website for scheduling.
What MSP Coverage Means for Rural Care
For rural seniors who travel to larger cities for specialist appointments — oncology in Bismarck, cardiology in Fargo, orthopedics in Grand Forks — QMB coverage pays every Medicare deductible and coinsurance charge at those providers. Without QMB, a single hospital stay can generate a $1,736 inpatient deductible that consumes months of Social Security income.
SLMB and QI cover the $202.90 monthly Part B premium, freeing up $2,434.80 per year. All three tiers trigger automatic Extra Help for covered Part D drugs at participating pharmacies.
The North Dakota Medicare Savings Programs & Extra Help Guide includes a document preparation checklist and income worksheet designed for rural households — with specific guidance on how mineral rights, farmland, and agricultural equipment are treated under ND HHS asset rules.
