North Dakota Extra Help: Medicare Part D Low-Income Subsidy Explained
What Extra Help Covers in 2026
Extra Help (officially the Part D Low-Income Subsidy) reduces or eliminates prescription costs for low-income Medicare beneficiaries. Under the Inflation Reduction Act, all Extra Help enrollees now receive the full subsidy — no more partial benefits.
In 2026, full Extra Help provides:
- Part D plan premiums covered up to the state benchmark level
- No annual prescription deductible
- Generic drug copays capped at $4.90 per prescription
- Brand-name drug copays capped at $12.15 per prescription
- $0 copays once you hit the catastrophic coverage threshold
The 2026 out-of-pocket prescription cap is $2,100, but with Extra Help your parent will rarely approach that number.
The Automatic Route: Apply for a Medicare Savings Program
The fastest path to Extra Help in North Dakota is getting approved for any Medicare Savings Program — QMB, SLMB, or QI. Enrollment in any of these programs automatically triggers deemed eligibility for Extra Help. CMS handles the data transfer; no separate application is needed.
ND HHS reports MSP approvals to CMS, which transmits deemed eligibility files to SSA and Part D plan sponsors. CMS mails a notice of deemed status (on purple paper) to the beneficiary.
Since MSP income limits are lower than standalone Extra Help limits, anyone who qualifies for a Medicare Savings Program automatically qualifies for Extra Help too.
Standalone Extra Help If MSP Doesn't Fit
If your parent's income or assets are too high for QMB, SLMB, or QI, they may still qualify for Extra Help on its own. The 2026 standalone limits are more generous:
| Single | Couple | |
|---|---|---|
| Monthly income | Up to $2,015 | Up to $2,720 |
| Resources | $18,090 | $36,100 |
Apply directly through the Social Security Administration at ssa.gov or by calling 1-800-772-1213. This is a federal application, separate from the state MSP process.
When someone applies for standalone Extra Help through SSA, the application data is electronically shared with ND HHS under the MIPPA agreement. The state treats it as an initiated MSP application and follows up for any missing information — so applying for Extra Help can also open the door to state premium assistance.

No State Pharmacy Program to Fall Back On
North Dakota does not operate a general State Pharmaceutical Assistance Program (SPAP). There's no state-funded prescription subsidy for seniors broadly. Prescription help comes through Extra Help (federal), the HIV/AIDS Drug Assistance Program (disease-specific), Indian Health Service facilities for eligible Native American beneficiaries, and manufacturer patient assistance programs.
This makes securing Extra Help through either the MSP pathway or the standalone application especially important for North Dakota seniors.
What Happens If MSP Coverage Ends
If your parent loses their Medicare Savings Program at annual recertification, deemed Extra Help doesn't disappear immediately. If coverage closes before July 1, Extra Help continues through December 31 of that year. If it closes on or after July 1, Extra Help extends through December 31 of the following year. That grace period provides time to apply for standalone Extra Help through SSA.
The Best Available Evidence Fix
If a pharmacy charges full price despite your parent being approved for Extra Help, you can invoke the CMS Best Available Evidence (BAE) policy. Provide the pharmacy or Part D plan with a copy of the official ND HHS approval letter or a printout from the Self-Service Portal. The plan must manually adjust copays at the point of sale.
The North Dakota Medicare Savings Programs & Extra Help Guide covers both the automatic MSP path and the standalone application process, with checklists for both routes.
