New Mexico SHIP Medicare Counseling for Seniors
What SHIP Actually Does in New Mexico
New Mexico's State Health Insurance Assistance Program is administered by the Aging and Disability Resource Center (ADRC) within the Aging and Long-Term Services Department (ALTSD), offering free one-on-one counseling to anyone on Medicare or approaching eligibility. SHIP counselors are trained volunteers and staff who help with plan comparisons, billing disputes, enrollment timelines, and the overlap between Medicare and Medicaid.
The ADRC also handles long-term care navigation and Medicaid waiver enrollment, while SHIP zeroes in on Medicare-specific questions — Part A hospital coverage, Part B outpatient, Part D prescription drugs, Medigap supplemental policies, and Medicare Advantage plans.
To reach a SHIP counselor, call the ALTSD helpline at 1-800-432-2080 and request Medicare counseling. Counselors are available at senior centers and Area Agency on Aging offices across the state.
When SHIP Counseling Matters Most for Long-Term Care Families
Medicare covers up to 100 days of skilled nursing care after a qualifying 3-day hospital stay — full coverage for days 1 through 20, then a daily copayment for days 21 through 100. After day 100, Medicare stops paying entirely. That cliff is where most families first confront the cost of long-term care in New Mexico, which runs $92,000 to $118,000 per year for private-pay nursing home care.
A SHIP counselor can help you understand exactly when Medicare coverage ends, what your copayment obligations are during days 21–100, and whether your parent's Medigap policy covers the gap. They can also clarify the difference between "observation status" and inpatient admission — a distinction that determines whether the 3-day rule is even triggered.
For families transitioning from Medicare-covered rehabilitation to Medicaid-funded long-term care, SHIP counselors explain how the two programs coordinate. Medicare Part B premiums, for instance, are deducted from the Medicaid applicant's income before calculating patient liability to a nursing facility.
SHIP vs. the ADRC: Which One to Call
If your question is about Medicare plans, enrollment periods, or billing, call SHIP. If your question is about Medicaid eligibility, the Turquoise Care Community Benefit, or the Central Registry waitlist for home care services, call the ADRC at the same number (1-800-432-2080) and specify what you need.
Many families need both. A parent entering a nursing home may start on Medicare-covered rehabilitation and then need to apply for Medicaid long-term care once the 100-day skilled nursing benefit expires. SHIP handles the Medicare side; the ADRC and your MCO care coordinator handle the Medicaid transition.

What SHIP Cannot Do
SHIP counselors do not provide legal advice. They cannot help you set up a Qualified Income Trust, structure an asset spend-down, or record a Transfer on Death deed to protect your parent's home from estate recovery. They also cannot represent you in a Medicaid denial appeal before the HCA Hearings Bureau.
For families navigating the full spectrum of long-term care financing — from the initial Medicare rehabilitation benefit through Medicaid eligibility and asset protection — a structured process guide fills the gap between free SHIP counseling and expensive elder law consultations. Our New Mexico Medicaid Long-Term Care & Asset Protection Guide walks through every step of that transition, including the financial thresholds, spousal protections, and estate recovery rules that SHIP counselors are not trained to address.
