Application Forms: HCA 100 vs. MAD 100

New Mexico offers two primary application forms for Medicaid. The HCA 100 (formerly the streamlined application) covers multiple programs simultaneously — Medicaid, SNAP, and LIHEAP — making it the default choice for most applicants. The MAD 100 is a Medicaid-specific form issued by the Medical Assistance Division, used primarily for long-term care and waiver benefit applications.

For long-term care Medicaid, either form works. The choice depends on whether your parent also needs SNAP benefits or energy assistance — the HCA 100 handles all three in one submission.

Use the current HCA 100 or MAD 100 application as directed by HCA or ISD. If the ISD requests a separate authorization or verification form during review, sign and return it with the requested records.

Filing Through YesNM

The YesNM portal (yes.state.nm.us) is New Mexico's online benefits application system. You can submit the HCA 100 or MAD 100 electronically, upload supporting documents, and check application status.

Practical tips for filing through YesNM:

• Create the account in the applicant's name if they are cognitively capable, or use your own credentials if you hold a valid Durable Power of Attorney with financial authority.
• Upload documents as clear, legible PDFs. Blurry phone photos of bank statements are a top cause of requests for additional information, which delay the 45-day processing timeline.
• Label each uploaded file clearly. The ISD reviewer processes dozens of applications — a file labeled "Chase checking Jan 2021–Dec 2025" is processed faster than "IMG_4532.jpg."
• Save the confirmation number. If the portal has a technical issue during submission, the confirmation is your proof of timely filing.

The CASA Scanning Center

If your parent cannot file electronically, paper applications and supporting documents can be mailed or hand-delivered to the Central ASPEN Scanning Area (CASA) in Bernalillo, New Mexico. CASA scans incoming paper documents into the state's electronic system and routes them to the appropriate ISD office for processing.

The CASA route adds processing time compared to direct YesNM portal submission. If you mail documents, use certified mail or a delivery service with tracking. Applications are date-stamped based on when CASA receives them, not when you mailed them — and the 45-day processing clock starts from the date the application is registered.

The 45-Day Processing Timeline

The ISD must issue an eligibility determination within 45 days of the application's registration. For applicants who need a disability determination — common for individuals under 65 applying through the Aged, Blind, and Disabled (ABD) track — the timeline extends to 90 days because the disability review requires a separate medical assessment.

If the ISD requests additional documentation during the review, respond by the stated deadline. Missing the documentation deadline can result in a denial for "failure to provide verification" — a denial that is entirely preventable.

Documents to Submit with the Application

The ISD will need all of the following for a long-term care Medicaid application:

• 60 months of bank statements for every account (checking, savings, money market, CDs)
• 60 months of investment account statements (brokerage, IRAs, 401(k)s)
• Current Social Security award letter
• Pension and annuity statements
• Most recent tax return
• Property deeds and mortgage statements
• Vehicle titles
• Life insurance policies with cash surrender values
• Proof of New Mexico residency (utility bill, lease)
• Birth certificate or proof of citizenship
• The executed Qualified Income Trust document (if income exceeds $2,982)
• Proof of any transfers, gifts, or property sales within the past 60 months
