New Mexico Medicaid Application Documents Checklist for Nursing Home
Filing a Medicaid long-term care application in New Mexico is an exercise in documentation. The Income Support Division reviews five years of financial records, and a single missing bank statement can delay the entire determination. Having everything organized before you file saves weeks of back-and-forth.
Where to File
New Mexico offers three filing channels:
- Online through the YesNM portal at yes.state.nm.us. You can upload supporting documents directly.
- By phone through the Income Support Division at 1-800-283-4465
- By mail using the HCA 100 or MAD 100 paper application form, sent to the Central Aspen Scanning Area (CASA) in Bernalillo, New Mexico. CASA is the state's centralized document intake facility — all mailed applications and supporting paperwork route through this single location.
Whichever channel you use, the application itself is either the HCA 100 (the streamlined multi-program application) or the MAD 100 (specific to Medicaid). Both cover long-term care.
The 45-Day Processing Window
Once the Income Support Division receives an application, they have exactly 45 calendar days to approve, deny, or delay the application. If the applicant is under 65 and a disability determination is required (because they don't have an existing SSA disability finding), the timeline extends to 90 days.
The clock starts when the application is received — not when it is complete. If ISD requests additional documents, the 45-day window continues ticking, but processing stalls until the missing items arrive. Filing with a complete packet from day one is the single most effective way to avoid delays.
Document Checklist
Identity and Residency
- Social Security cards for the applicant (and spouse, if married)
- Government-issued photo ID (driver's license, state ID, or passport)
- Proof of New Mexico residency (utility bill, lease, or property tax statement)
- Birth certificate or proof of age
- Proof of U.S. citizenship or immigration status
Income Verification
- Social Security benefit award letter (the most recent annual statement)
- Pension statements showing gross monthly amount
- Annuity contracts and payment schedules
- VA benefit award letters
- Rental income documentation
- Interest and dividend statements from all investment accounts
Asset Documentation (60 Months)
- Bank statements for every checking, savings, money market, and CD account — the full 60 months preceding the application date. This includes accounts that were open during the lookback period but have since been closed.
- Investment account statements — brokerage, mutual fund, and stock accounts for 60 months
- Retirement account statements — IRAs and 401(k)s. In New Mexico, these are fully countable resources regardless of distribution status.
- Life insurance policies — face amount and current cash surrender value for each policy
- Vehicle titles — one vehicle is exempt; documentation for any additional vehicles
- Real estate deeds — for all properties, including the primary home
- Burial fund documentation — statements for any designated burial accounts and copies of prepaid funeral contracts
Qualified Income Trust
- A notarized copy of the executed Qualified Income Trust (Miller Trust) document, if the applicant's gross monthly income exceeds $2,982
- The dedicated QIT bank account number and routing information
- Proof that the trust has been funded (initial deposit statement)
Medical Documentation
- Physician certification of the need for nursing facility level of care
- Current medical records documenting ADL assistance requirements
- Hospital discharge summary (if the application follows an acute hospitalization)

Common Reasons for Delays
The most frequent application problems are incomplete financial records. The ISD will request clarification or additional documents if:
- Bank statements have gaps in the 60-month history
- Large deposits or withdrawals are unexplained
- A closed account has no closing statement
- The Qualified Income Trust document is missing or improperly drafted
- Real estate ownership records are unclear
Each document request adds days or weeks to the timeline. If you know a particular transaction will raise questions — a large gift to a family member, a property sale below market value, a retirement account rollover — prepare a written explanation and supporting documentation before filing.
