What Documents Are Needed for a QMB Application in Nevada?
Incomplete Applications Are the Number One Cause of Delays
DWSS has 45 days to process a Medicare Savings Program application. When documents are missing, DWSS mails an Information Request (sometimes Form 2429) giving you 10 days to respond. Miss that window and the application can be denied for non-cooperation — not because your parent did not qualify, but because the paperwork was incomplete.
Gathering everything before you submit saves weeks.
The Verification Checklist
Medicare entitlement: A copy of the red, white, and blue Medicare card or an SSA award letter showing Part A and Part B enrollment. DWSS needs this to confirm your parent is a Medicare beneficiary.
Identity and Nevada residency: A Nevada Driver's License or State ID card. For residency, a utility bill, lease agreement, or property tax notice showing a Nevada address.
Income documentation: The most recent Social Security Benefit Statement (Form SSA-1099 for annual, or the monthly benefit statement from ssa.gov). If your parent receives a pension or annuity, include the most recent statement showing gross monthly amount. For part-time wages, include pay stubs covering the last 30 days.
DWSS counts gross income before any deductions — including the Medicare Part B premium that Social Security withholds. Do not submit the net bank deposit amount as income proof; DWSS needs the gross figure.
Bank and financial account statements: Statements from every checking account, savings account, and certificate of deposit covering the preceding calendar month. The federal resource standard is $9,950 for an individual and $14,910 for a couple, but the Nevada DWSS B-300 manual prints $9,430 and $14,130. If your parent's resources are near either threshold, verify which figure DWSS is applying.
Jointly held accounts are presumed owned entirely by the applicant unless you provide documentation (like bank signature cards) showing proportional contributions.
Citizenship or immigration status: U.S. Passport, birth certificate, Certificate of Naturalization, or Permanent Resident Card (Form I-551).
What Counts as a Resource — and What Does Not
The resource limit catches many families off guard. But Nevada's exempt assets list is more generous than most people realize:
Exempt (does not count):
- Primary residence, as long as your parent lives there or intends to return
- One vehicle, regardless of value
- Household goods and personal effects
- Prepaid burial funds up to $1,500 (or an irrevocable burial contract of any value)
- Life insurance policies with total face values under $1,500
Counted:
- Cash, checking, savings, CDs, money market accounts
- Stocks, bonds, mutual funds
- Additional vehicles (equity value)
- Non-exempt real property (equity value, unless actively listed for sale at fair market value)
For a life insurance policy with a total face value at or above $1,500, ask DWSS how it will be counted before deciding whether it affects eligibility.

If You Get Form 2429
The Information Request notice gives you 10 calendar days to respond. Do not wait to gather perfect documentation. Submit what you have by fax (Southern Nevada: 702-486-1634, Northern Nevada: 775-448-5094) and note on the fax cover sheet which items you are still obtaining. If you cannot obtain every document by the deadline, contact DWSS before the deadline; a partial response does not guarantee an extension.
Keep your fax confirmation page or certified mail receipt as a record that can help if DWSS later says it did not receive your response.
The Nevada Medicare Savings Programs & Extra Help Guide includes the full document preparation checklist with specific guidance on what DWSS accepts, how to handle joint bank accounts, and what to do when an Information Request arrives.
