myCase Utah: How to Apply for Medicare Savings Online
The myCase portal at jobs.utah.gov/mycase is DWS's primary online system for applying to Medicare Savings Programs in Utah. It's the fastest route — but the interface can be confusing if you don't know which sections to complete.
Setting Up Access
You'll need a UtahID account to log in. Go to id.utah.gov to create one if you don't have one yet. UtahID is the state's single sign-on system used across multiple Utah government services.
If you're an adult child applying for a parent, you can create your own UtahID and link it to your parent's case after filing Form 114AR (the DWS authorization form). This lets you view their case status, upload documents, and receive notices through your own login.
Starting a Medical Assistance Application
Once logged in:
- Select "Apply for Benefits"
- Check "Medical Assistance" — this covers all Medicaid and Medicare Savings Program applications
- Work through the eligibility questions about household composition, income, and assets
- The system will ask for basic identifying information, Social Security numbers, and current income sources
You don't need to specify QMB, SLMB, or QI. DWS evaluates your answers against all three programs and assigns the right level.
Uploading Verification Documents
myCase lets you upload documents electronically — photographs of paper documents work fine. Upload:
- Social Security Award letter (showing gross monthly benefit)
- Bank statements covering the last 30–60 days
- Medicare card or SSA letter confirming Part A entitlement
- Proof of Utah residency
DWS checks SSA income databases and the state Asset Verification System electronically. If everything matches, they may not request additional paperwork. But having documents uploaded proactively can prevent the 10–15 day verification request delay.

Tracking Your Application
After submission, myCase shows the current status of your application. You can see whether DWS has requested additional documents, whether the application is pending review, and when a decision has been issued. Approval and denial notices also appear in the portal, though DWS sends paper copies by mail as well.
Common Stumbling Points
The form covers everything. Form 61 (which myCase walks you through digitally) is the universal DWS application for SNAP, financial assistance, child care, and medical assistance. The MSP-related sections are a fraction of the full form. If you see questions about food assistance or child care, you can skip those sections — you're applying for medical assistance only.
The portal doesn't explain what happens next. After DWS approves an MSP, the buy-in data goes to CMS and then to Social Security. The Part B premium deduction from your parent's check continues for 30–60 days while that exchange processes. myCase won't show that lag — it just shows the MSP as approved.
Renewals also go through myCase. DWS recertifies MSP eligibility every 12 months. They attempt an automated renewal first using electronic databases, but if they can't verify everything automatically, they'll send a renewal packet. If you've authorized a representative through Form 114AR, you'll see renewal notices in your own myCase login.
When myCase Isn't the Best Route
For rural residents with limited internet access, phone (1-866-435-7414) or paper applications work just as well. The paper Form 61 can be submitted by mail or fax, or dropped off at any DWS Employment Center.
The Utah Medicare Savings Programs & Extra Help Guide provides section-by-section guidance for the myCase application, including which fields matter for MSP-only applicants and how to handle verification requests.
