Mississippi QMB Premium Refund: How to Get Your Part B Money Back

Why Premiums Keep Coming Out After Approval

Your parent got the DOM approval letter confirming QMB or SLMB. But the next Social Security check may still show a deduction for the standard 2026 Part B premium of $202.90. And the one after that. This is normal — frustrating, but normal.

Here's the chain of events that has to happen before the deduction stops:

  1. DOM issues the approval and sends an electronic accretion file to CMS through the Benefit Exchange Network (BENDEX).
  2. CMS processes the accretion and notifies SSA.
  3. SSA updates the Master Beneficiary Record to reflect state buy-in.
  4. SSA stops deducting the Part B premium from the monthly Social Security payment.
  5. SSA calculates and issues a lump-sum refund for all premiums deducted after the coverage start date.

DOM advises allowing four to six weeks for the Part B premium to be added back to your Social Security check. During that period, the premium may continue to come out of Social Security.

How the Refund Arrives

Once SSA completes the buy-in processing, the refund comes as a separate payment — either a check mailed to your parent's address or a direct deposit if that's how they receive Social Security. It is not folded into the regular monthly payment.

The refund covers every Part B premium deducted from the MSP effective date forward. For QMB, coverage starts the first day of the month after approval (prospective only). For SLMB and QI, retroactive coverage can go back up to three months before the application month — meaning the refund can cover premiums for those retroactive months too.

When to Start Worrying

If 90 days pass from the DOM approval date with no premium adjustment and no refund:

  1. Call SSA at 1-800-772-1213. Have the DOM approval notice ready with the effective date and coverage group.
  2. Ask SSA to verify the buy-in accretion date — the date DOM's electronic file was received and processed.
  3. If SSA has no record of the accretion, contact your DOM regional office and ask them to retransmit the buy-in file.

The bottleneck is almost always on the data exchange side. DOM approved the case, but the electronic file either wasn't sent, wasn't received, or is sitting in a processing queue.

Special Cases: Direct-Pay and Railroad Retirement

Most Medicare beneficiaries pay Part B through Social Security withholding. But some pay CMS directly via quarterly invoices — typically people who don't receive monthly Social Security payments, or whose benefits are too small for the premium to be withheld.

If your parent pays Part B directly, the buy-in adjustment comes as a credit from CMS rather than a Social Security refund. The timeline is similar, but the credit appears on the quarterly invoice rather than as a separate payment.

Railroad Retirement Board (RRB) beneficiaries receive their premium adjustment directly from the RRB, following the same BENDEX data exchange process.

Tracking Every Dollar

During the waiting period, keep a log of every monthly deduction: the payment date, the gross Social Security amount, the Part B deduction, and the net deposit. When the refund arrives, compare the total refunded against the total deducted from the MSP effective date forward. They should match exactly.

If there's a discrepancy — SSA refunded three months of premiums but four were deducted after the effective date — call SSA with the documentation and request a correction.

The Mississippi Medicare Savings Programs & Extra Help Guide includes a premium refund tracker designed for exactly this purpose, with columns for every data point SSA needs if you have to dispute the refund amount.
