Medicare Savings Program Texas Application — How to Apply Step by Step

Pick the Right Form Before You Start

The most common mistake families make is downloading Form H1200 — the full 19-page HHSC application designed for nursing home Medicaid, long-term care waivers, and every other MEPD program. It asks for five years of asset transfer history, annuity details, and burial contract documentation that your parent does not need to provide for a Medicare Savings Program.

The correct form for an MSP-only application is Form H1200-EZ (Application for Assistance — Aged and Disabled). It's shorter, focused on current income and resources, and doesn't ask the questions that trip families up or cause unnecessary processing delays.

Four Ways to Submit

Texas HHSC accepts MSP applications through four channels:

  1. Online: YourTexasBenefits.com — create an account, select "Apply for Benefits," and submit documents digitally. This is the fastest route and lets you track case status in real time.
  2. Mobile app: Your Texas Benefits app — works the same as the website, with the added convenience of using your phone camera to upload documents.
  3. Phone: Call 2-1-1 and select Option 2 for the MEPD unit. HHSC staff can take a telephonic application and process a verbal signature.
  4. Mail: Send the completed Form H1200-EZ to PO Box 149024, Austin, TX 78714-9024. Families in rural counties without a local HHSC office often use this route, and mail is sent to HHSC's Austin processing address.

Documents You Need

Gather these before you start — missing items trigger a Form H1020 (Request for Information) notice with a strict 10-day deadline:

  • Medicare card showing Part A and Part B effective dates
  • Income proof: Social Security benefit statement (Form SSA-1099 or the annual COLA notice), pension vouchers, annuity statements
  • Bank statements: complete most-recent-month statements for all checking and savings accounts, including all pages
  • Texas ID: driver's license, state ID, or a recent utility bill showing the Texas residential address
  • Citizenship or immigration status: passport, birth certificate, or Permanent Resident Card (Form I-551)

If you're applying on behalf of a parent, you also need Form H1003 (Appointment of an Authorized Representative to Allow Another Person to Act for You) or a legal power of attorney.

What Happens After You Submit

HHSC has 45 calendar days to issue a decision. If documentation is missing, they send Form H1020 with a 10-day deadline to provide the missing items. Failure to respond results in a procedural denial — though your parent can reapply immediately.

Once approved, HHSC issues Form H1009 (Notice of Eligibility) detailing the approved MSP tier, coverage effective dates, and income calculations. For QMB, coverage starts the first day of the month after the approval month. For SLMB and QI, coverage is effective the first of the application month, with up to three months of retroactive coverage.

If the SSA Said "Apply for Extra Help First"

Applying for Extra Help at Social Security sends information to Texas HHSC to initiate MSP screening. It does not approve Texas MSP benefits; HHSC still verifies eligibility. Respond to HHSC requests for information and, if no notice arrives, submit Form H1200-EZ directly.

Our Texas Medicare Savings Programs & Extra Help Guide includes a section-by-section walkthrough of Form H1200-EZ, a document checklist you can print and check off, and fillable trackers for monitoring your application timeline and HHSC correspondence.
