MassHealth Buy-In Processing Time: How Long to Get Approved

You mailed the Medicare Savings Program application three weeks ago, and the only response so far is silence. That is normal — but it helps to know the timeline, what can slow things down, and when to escalate.

The Standard Timeline

MassHealth operates under a federal 45-day standard for processing non-disability Medicaid and MSP applications. That standard is just over six weeks.

After MassHealth approves the application, a second clock starts. The state transmits eligibility records to CMS, which updates the Medicare Beneficiary Database and notifies the Social Security Administration. This multi-agency handoff typically takes 60 to 90 days. Continue allowing the Part B deduction or paying direct Medicare bills during that window. Premiums paid for months at or after the buy-in effective date are refunded retroactively once processing completes.

In total, from the day you submit the application to the day your parent sees a larger Social Security deposit, plan for roughly three and a half to four and a half months if MassHealth uses the full 45-day processing period and the handoff takes its typical 60 to 90 days.

What Causes Delays

Unnecessary verification requests. MassHealth's system sometimes generates a Request for Information (RFI) asking for bank statements or asset documentation — even though the MSP asset test was eliminated in March 2024. If this happens, call MassHealth Customer Service at 1-800-841-2900 and clarify that the application is MSP-only. Do not wait for the RFI deadline to pass, because unanswered RFIs can lead to a denial for failure to verify.

Missing Medicare information. If the application does not include a Medicare claim number or MBI, MassHealth cannot initiate the buy-in process. Double-check that the Medicare card number is on the form.

Incomplete income documentation. MassHealth verifies income through electronic data matching, but discrepancies between what you reported and what their systems show will trigger a verification request. Submitting a current SSA-1099 or benefit verification letter with the application reduces this risk.

Filing the wrong form. Submitting the comprehensive SACA-2 instead of the dedicated MHBI form adds processing steps. MassHealth will still screen for MSP eligibility, but the longer form invites unnecessary asset-related inquiries.

When to Escalate

If 45 days have passed without any notice — approval, denial, or request for additional information — take these steps:

  1. Call MassHealth Customer Service at 1-800-841-2900 and ask for a status update. Have your filing date ready (the fax confirmation timestamp, certified mail receipt, or portal upload confirmation number).
  2. Request a formal status escalation. Ask the representative to note the call and the 45-day deadline in the case record.
  3. Follow up in writing. Fax a brief letter to the Charlestown MEC at (617) 887-8799 stating the application filing date and requesting expedited processing.

If MassHealth denies the application, the denial notice includes instructions for requesting a Fair Hearing before the MassHealth Board of Hearings. The deadline is 30 days from the date on the notice.

Keep Paying During the Wait

Until the buy-in accretion process is complete and SSA stops deducting the Part B premium, your parent should continue paying Medicare directly or allowing the Social Security deduction. Letting premiums lapse can create a gap in Medicare coverage. Every dollar paid during this interim period is fully refunded once the buy-in takes effect.

The Massachusetts Medicare Savings Programs Guide includes a premium refund tracker and a timeline checklist that maps each step from application through the SSA reimbursement, so you know exactly what to expect at each stage.
