MassHealth Buy-In Premium Refund: How Retroactive Reimbursement Works

Your parent was approved for MassHealth Buy-In, but Social Security is still deducting the $202.90 Part B premium every month. The refund is coming — it just moves through three separate federal agencies before it arrives.

How the Refund Process Works

When MassHealth approves a Medicare Savings Program application, the state initiates a buy-in accretion — an electronic record transmitted to CMS. Here is what happens in sequence:

  1. MassHealth transmits the eligibility record to CMS, including your parent's Medicare claim number and the coverage start date.
  2. CMS updates the Medicare Beneficiary Database, confirming that the state is now responsible for the Part B premium.
  3. CMS notifies SSA through the Benefit Exchange Network (BENDEX) to stop withholding the Part B premium from your parent's Social Security check.
  4. SSA processes a lump-sum reimbursement for all Part B premiums deducted from the check on or after the buy-in effective date.

The refund is issued as a direct deposit or a paper check from the U.S. Treasury.

The Timeline

The multi-agency handoff between MassHealth, CMS, and SSA typically takes 60 to 90 days from the date of state approval. Continue allowing the Part B deduction or paying direct Medicare bills during this interim period while the systems reconcile.

As part of that reconciliation, SSA stops the deduction and processes a lump-sum reimbursement for eligible months. For three eligible months, the refund would be roughly $609 (three months at $202.90).

QMB vs. SLMB/QI Refund Differences

The refund amount depends on which MSP tier was approved and when coverage starts:

QMB (Senior Buy-In): Coverage begins on the first day of the month following the approval date. Refunds cover premiums deducted from that date forward. QMB is prospective only — no retroactive coverage is available.

SLMB and QI (Buy-In): Coverage can begin on the first day of the month the application was received, and may extend retroactively up to three months before the application month if the applicant was eligible during those months. This means the lump-sum refund for SLMB or QI can cover up to three additional months of premiums — potentially over $800 in a single payment.

Beneficiaries Who Pay Medicare Directly

Some seniors pay Medicare Part B premiums through quarterly bills (CMS-500 invoices) rather than Social Security deductions — typically those who do not receive Social Security benefits. For these beneficiaries, CMS cancels outstanding invoices once the buy-in accretion is processed. If bills were already paid for months covered by the buy-in, CMS issues a refund directly.

What to Do If the Refund Is Late

If 90 days have passed since the approval date and the Social Security deduction has not stopped:

  1. Check your parent's MassHealth portal or call MassHealth at 1-800-841-2900 to confirm the buy-in accretion was transmitted.
  2. Call SSA at 1-800-772-1213 and ask whether the buy-in record has been received and processed.
  3. If SSA has not received it, ask MassHealth to retransmit the accretion file.

Keep the MassHealth Notice of Decision showing the approval date and coverage start date. This is the key document for proving which months should be refunded.

Do Not Stop Paying While You Wait

It is tempting to stop the Social Security deduction or skip a Medicare bill while the application processes. Do not do this. Part B can be terminated for nonpayment. If that happens, the next chance to enroll may be the January 1 through March 31 General Enrollment Period, and coverage starts the month after enrollment. A late-enrollment penalty may also apply for as long as the person has Part B. Pay now, get refunded later.

The Massachusetts Medicare Savings Programs Guide includes a premium refund tracking worksheet that maps each step of the accretion process and calculates the expected refund amount based on your parent's approval date and tier.
