iServe Nebraska Medicare Savings Application: Portal Guide

Nebraska moved its Medicaid and Medicare Savings Program applications from ACCESSNebraska to iServe Nebraska, and the transition left a trail of confused applicants. The old portal links still float around the internet, the new dashboard organizes things differently, and nobody published a walkthrough for seniors or their adult children filing for QMB, SLMB, or QI. Here's what actually matters.

Creating an iServe Account

Go to iserve.nebraska.gov and register. You need a valid email address, a phone number for verification, and basic identity information. If you're an adult child applying for a parent, create the account in the parent's name — the applicant is the benefit recipient. You'll designate yourself as an authorized representative within the application itself or by attaching Form MLTC-35.

Once registered, log in and select "Apply for Benefits." Navigate to the Medical Assistance intake section. The system will ask about the type of coverage — choose the path for aged and disabled programs, which is where Medicare Savings Programs live.

Uploading Documents

iServe accepts PDF and image uploads directly within the application. Upload each document as a separate file with a clear filename — "SSA_Award_Letter_2026.pdf" is easier for the eligibility worker than "scan_003.jpg."

Critical uploads for an MSP application:

  • Social Security benefit verification letter showing gross monthly benefit
  • Bank statements for every account (full calendar month prior to application)
  • Medicare card or MBI entitlement notice
  • Signed MLTC-35 authorized representative form (if someone else is managing the case)

Upload everything with the initial submission. Waiting for DHHS to request documents adds 10 to 14 days to processing — and if you miss that window, the case gets closed.

Monitoring Action Alerts

After submission, check the iServe dashboard's Action Alerts regularly. This is where DHHS posts verification requests, notices of missing documents, and decision letters. There is no push notification system — you have to log in and look.

If DHHS needs additional information, the alert will specify what's missing and set a deadline. Respond by uploading the requested document directly through the portal. Keep a screenshot or PDF of each upload confirmation.

Renewals Through iServe

MSP eligibility is redetermined every 12 months. DHHS first attempts an automated renewal using electronic data from SSA and other federal sources. If automated verification works, your coverage renews without any action from you.

If it doesn't, DHHS mails a pre-populated renewal form. You can also check iServe for a renewal action alert. Complete and return the form with updated financial documentation within 30 days.

If MSP coverage lapses because you missed the renewal deadline, Nebraska gives you a 90-day grace period to submit late paperwork without filing a brand-new application. After 90 days, you start over.

Common Portal Issues

The iServe interface occasionally loops between pages — particularly when switching between the legacy ACCESSNebraska sections and the newer dashboard. If this happens, clear your browser cache, use a different browser, or try a private/incognito window.

For phone-first seniors who find the portal difficult, the alternative routes remain open: call (855) 632-7633 to apply verbally, or mail Form MLTC-64 to the DHHS Document Imaging Center at P.O. Box 2992, Omaha, NE 68103-2992.

The Nebraska Medicare Savings Programs & Extra Help Guide includes step-by-step iServe navigation instructions, a document upload checklist, and a tracking log to monitor your application from submission through approval.
