Illinois Medicare Savings Program Renewal: How Redetermination Works and How to Keep Coverage
Two Renewal Pathways
Illinois redetermines Medicare Savings Program eligibility every 12 months. The renewal process follows one of two tracks — and which one applies depends on whether the state can verify your continued eligibility electronically.
Ex parte automated renewal — the preferred track. The Integrated Eligibility System (IES) cross-checks state data sources (AWVS, SSA BENDEX) against your current eligibility criteria. If everything confirms — income, resources, Medicare enrollment — coverage renews automatically. You receive Form HFS 2381C confirming continued eligibility. No paperwork required, no response needed.
Manual form renewal — triggered when electronic data is insufficient or incomplete. IDHS mails a renewal packet 45–60 days before your certification end date. Single-program medical cases receive Form HFS 643M. Households receiving SNAP or cash assistance alongside medical coverage get Form HFS 1893.
The manual renewal requires a signed response and verification documents (updated bank statements, income documentation). Missing the deadline triggers a procedural closure — your MSP coverage ends.
How to Know Which Track You're On
You won't know in advance. The automated renewal happens silently — the first sign is Form HFS 2381C arriving in the mail confirming continuation. If a manual renewal packet shows up instead, that's your signal to act.
Check the Manage My Case (MMC) section of the ABE portal regularly during the months leading up to your certification anniversary. Pending renewal actions appear there, and the portal reflects the deadline.
For adult children managing a parent's case remotely, setting a calendar reminder for 60 days before the certification end date catches the renewal window regardless of which track applies. If no renewal packet arrives and no action appears in MMC, the automated renewal likely handled it — but calling the FCRC at 1-800-843-6154 to confirm is worth the two-minute phone call.
The 90-Day Grace Period
If your parent misses the manual renewal deadline and coverage closes, there's a 90-day reconsideration window under federal rules (42 CFR § 435.916). Submit the completed renewal packet with all required verifications within 90 days of the closure date, and the FCRC must process the renewal without requiring a brand-new application.
If eligible, coverage reinstates retroactively to the date it closed — no gap in protection.
After 90 days, a new application is required from scratch.

Reporting Changes Between Renewals
Between annual renewals, you're required to report changes in income, resources, household composition, or address within 10 calendar days. Common changes that affect eligibility:
- A pension increase or decrease
- A new source of income (part-time employment, inheritance)
- A spouse moving in or out of the household
- Selling property or closing/opening bank accounts
- Moving to a new address within Illinois
Report through the ABE portal's Manage My Case section, by calling the IDHS helpline, or by notifying your county FCRC directly.
The COLA Disregard: January–March Protection
Every January, Social Security's Cost-of-Living Adjustment raises benefit amounts. This can push a beneficiary's gross income above MSP limits before the new Federal Poverty Level guidelines take effect (usually in April).
Illinois prevents this gap from terminating coverage. The annual COLA increase is disregarded for QMB, SLMB, and QI-1 eligibility during January, February, and March. Updated FPL income standards are implemented in April, restoring the alignment between COLA-adjusted income and eligibility thresholds.
During this window, you don't need to do anything. The disregard is applied systemically — no request or documentation required.
What to Do If Coverage Lapses
If you receive a termination notice (Form IL444-0360C) for a renewal failure:
- Within 10 days — file an appeal to preserve existing benefits under "aid paid pending" status while the appeal proceeds
- Within 60 days — file an appeal to contest the termination through a fair hearing
- Within 90 days — submit the renewal documents for reconsideration without needing a full new application
- After 90 days — file a brand-new application through ABE, paper, or phone
The fastest resolution for a procedural closure (termination due to missing paperwork, not an actual eligibility change) is submitting the renewal documents within the 90-day window. If the termination was based on an incorrect income or resource determination, filing a formal appeal within 60 days — and within 10 days for aid paid pending — is the stronger path.
