How to Apply for the Medicare Savings Program in Pennsylvania
Four Ways to File a Pennsylvania MSP Application
Pennsylvania accepts Medicare Savings Program applications through four channels. You don't need all four — pick whichever fits your situation:
1. COMPASS online portal — Submit through compass.dhs.pa.gov or the myCOMPASS PA mobile app. This is the fastest route if you're comfortable uploading documents digitally. You can photograph bank statements and pay stubs and attach them directly.
2. In person at your County Assistance Office — Submit a completed paper application directly to the CAO for your county.
3. By phone — Call the DHS Consumer Service Center at 1-866-550-4355 to start an application over the phone.
4. Through Social Security (MIPPA transfer) — File Form SSA-1020 (the Extra Help application) with Social Security, and SSA automatically transmits your data to Pennsylvania DHS under federal law. The date Social Security receives your Extra Help application becomes your official MSP filing date in Pennsylvania — no separate state application is required.
Which Form Do You Need?
For Medicare Savings Programs specifically, file Form PA 600 M (Application for Medicare Buy-In). This is the state form used to apply for MSP benefits.
Don't confuse it with Form PA 600 (the general Medical Assistance application) or PA 600 L (for long-term-care services). Use those forms when you also need full Medical Assistance or long-term-care waiver services.
Documents to Gather Before You Apply
The County Assistance Office verifies some data electronically — Social Security benefit amounts, Medicare entitlement dates, and disability status come through direct federal data feeds. Be ready to provide paper verification if electronic matches reveal discrepancies. Keep these records available:
- Bank statements covering the most recent month (all accounts)
- Private pension pay stubs or benefit letters
- Health insurance premium statements
- Proof of Pennsylvania residence (utility bill, lease, or similar)
Electronic verification sometimes catches discrepancies with federal records. Having paper documentation ready prevents processing delays when the CAO requests clarification.

The 45-Day Processing Clock
Under 55 Pa. Code § 141.64, the County Assistance Office must process your application within 45 calendar days from the date they receive it. If you're past that window without a decision, escalate in this order:
- Contact your assigned CAO caseworker or their supervisor directly
- Call the DHS Consumer Service Center at 1-866-550-4355
- Reach out to PA MEDI (1-800-783-7067) or the Pennsylvania Health Law Project helpline (1-800-274-3258)
- File an administrative appeal based on agency inaction
Applying on a Parent's Behalf
Family relationship alone doesn't authorize you to manage someone's case. You need explicit authorization filed with each agency:
- PA DHS: Form PA 1847 (Representative Authorization) or a copy of a durable power of attorney for financial affairs
- Social Security: Form SSA-1696 (Appointment of Representative)
- PACE/PACENET: A separate signed PACE Authorization Form or verified POA
Get these filed before or alongside the MSP application. Without them, the CAO cannot discuss case details with you — even as a son or daughter.
What Happens After Approval
Once approved, the CAO issues Form PA/FS 162 with your program tier (QMB, SLMB, or QI), effective coverage date, and appeal rights. For SLMB and QI, coverage can be retroactive up to three months before your application date if you met the income requirements during those months.
The Part B premium stops coming out of your Social Security check — but expect a 60- to 90-day lag while the state's electronic records sync with SSA. During that window, deductions continue. Once the sync completes, SSA deposits a lump-sum reimbursement for every month of back-deducted premiums.
