$0 Colorado — Medicare Savings Program Eligibility Checklist

How to Apply for Medicare Savings Program Colorado

Colorado doesn't have a separate application for Medicare Savings Programs. That's the first thing most families get wrong — they spend hours looking for a dedicated QMB or SLMB form that doesn't exist.

The Application Is the Standard Medical Assistance Form

Whether you apply online through PEAK or on paper, you're filling out Colorado's general Medical Assistance application. The Colorado Benefits Management System (CBMS) automatically evaluates every medical assistance applicant for QMB, SLMB, and QI-1 eligibility based on the income and asset information submitted. No checkbox to tick, no separate program to select.

This means the same application can determine eligibility for full Health First Colorado (Medicaid) and an MSP tier. Approval for QMB, SLMB, or QI-1 also establishes deemed eligibility for Extra Help through CMS.

Applying Through Colorado PEAK (Online)

PEAK is the fastest route. Here's the actual process:

  1. Go to peak.my.state.co.us and create a user account
  2. Select "Apply for Benefits" then "Medical Assistance"
  3. Complete the household, income, and asset sections — enter gross Social Security income before any deductions
  4. Upload verification documents through the web uploader or PEAK Health mobile app
  5. Sign electronically and record your Application Tracking Number

The PEAK interface won't explicitly mention "Medicare Savings Program" during the application flow. Don't let that throw you. As long as you've entered Medicare enrollment information and income details, CBMS runs the MSP evaluation automatically.

Applying on Paper

Download the Application for Public Assistance (Form PA-1) or the simplified Medical Assistance Application from the HCPF website. Submit the completed form to the Department of Human Services in your parent's county of residence — by mail, in person, or through county drop boxes.

Key county offices:

  • Denver County: Denver Department of Human Services, 1200 Federal Blvd, Denver, CO 80204
  • El Paso County: 1675 W. Garden of the Gods Rd, Colorado Springs, CO 80907
  • Montrose County (Western Slope): 1845 S. Townsend Ave, Montrose, CO 81401

You can also initiate an application by calling the Health First Colorado Member Contact Center at 1-800-221-3943.

Documents You'll Need

Gather these before starting:

  • Medicare card or SSA Benefit Verification Letter showing Part A/B start dates
  • Social Security Award Letter showing gross monthly benefit amount (before Part B deduction)
  • Bank statements for checking, savings, and investment accounts — most recent month
  • ID verification — driver's license, passport, or birth certificate (only if CBMS can't verify electronically through SSA)

Don't submit net Social Security amounts. Colorado needs gross income — the full amount before the $202.90 Part B premium is deducted. County workers then apply the standard $20 unearned income disregard during their CBMS calculation.

What Happens After You Apply

County human services has 45 calendar days to process the application. During that window:

  • If documents are missing, you'll get a Request for Information with at least 10 days to respond
  • If everything is complete, CBMS runs the eligibility determination and generates a Notice of Action
  • QMB coverage starts the first day of the month after approval — it's never retroactive
  • SLMB and QI-1 coverage can be retroactive up to three months before the application month

That prospective-only rule for QMB is why filing quickly matters. Every month of delay is another $202.90 gone from the check, plus whatever deductibles and copays stack up.

Helping a Parent From Out of State

If you're managing this for a parent remotely, PEAK is your primary tool. But there's a critical distinction: filling out the form as a "preparer" doesn't give you authority to receive notices or talk to caseworkers about the case.

For full case management authority, you and your parent need to complete HCPF's Authorized Representative Designation Form. This grants you the right to receive Notices of Action, complete renewals, upload documents, and represent your parent at hearings.

Note that this state form doesn't carry over to Social Security — SSA requires its own Form SSA-1696 or Power of Attorney for Medicare and benefit discussions.

Our Colorado Medicare Savings Programs & Extra Help Guide includes the complete document checklist, a PEAK walkthrough with screenshots, and the authorized representative setup process.
