How to Help a Parent Apply for Med-QUEST Medicare Savings in Hawaii

Why Adult Children End Up Managing This

Most Medicare Savings Program applications in Hawaii are driven by adult children, not the seniors themselves. A parent's Social Security check drops by $202.90 and they mention it on the phone. Or a Med-QUEST renewal notice arrives and the parent isn't sure what to do with it. Or a doctor's office sends a bill that shouldn't exist.

The child starts researching, discovers QMB or SLMB exists, and realizes the application has to go through Med-QUEST — a state agency the parent may never have dealt with directly.

Who Can Sign the Application

The DHS 1100 application requires the applicant's signature. An adult child can fill out every section — gather income figures, compile bank statements, complete the form — but the senior applicant must sign unless they've been adjudicated legally incapacitated.

There's no shortcut through a general Power of Attorney. A standard Hawaii POA doesn't automatically grant authority to handle Med-QUEST matters. Neither does being an SSA Representative Payee.

Setting Up as an Authorized Representative

To fully manage your parent's Med-QUEST case — submit applications, receive notices, speak with eligibility workers, and represent your parent at hearings — you need Form DHS 1127, "Appointment of Authorized Representative."

Your parent signs this form designating you (or another person) as their authorized representative. Once on file with Med-QUEST, you can:

  • Submit applications and renewal paperwork on their behalf
  • Receive copies of all official notices (including termination and renewal notices)
  • Call Med-QUEST and discuss the case with eligibility workers
  • Represent the applicant at fair hearings

Without DHS 1127, Med-QUEST can't discuss case details with you due to privacy rules. You can still help physically fill out forms, but the parent must be the one to call or sign.

Managing the Application From the Mainland

If you live in California, Washington, or anywhere on the mainland, the practical challenges are real: you're 2 to 6 hours ahead of Hawaii Standard Time, you don't have physical access to original documents, and time-sensitive mail arrives at an address you can't check.

Strategies that work:

Get DHS 1127 signed early. Mail the form to your parent, have them sign it, and submit it to Med-QUEST before you start the application. This lets you handle everything from your end.

Use the KOLEA portal. The online system at medical.mybenefits.hawaii.gov allows remote document uploads. You can create an account, link it to your parent's case (with authorized representative status), and upload scanned documents without mailing anything.

Photograph or scan documents locally. Ask your parent (or a local family member) to create clear copies of the Medicare card, Social Security benefit letter, bank statements, and Hawaii ID for submission through KOLEA or another Med-QUEST channel.

Fax as backup. Each island's Med-QUEST office has a fax number. Fax the completed DHS 1100 and DHS 1100B with a cover sheet to the relevant office, and keep the fax confirmation.

Set a mail watch for renewal notices. The annual renewal form (N-14 / DHS 1100B-2) has a 30-day deadline. Ask a local relative to watch for state mail or consider USPS Informed Delivery to get email scans of incoming mail at your parent's Hawaii address.

What the Application Requires

Whether you file in person, online, or by mail, the documentation is the same:

  • Medicare card (showing Part A and/or Part B enrollment)
  • Social Security benefit letter (most recent, showing gross monthly amount)
  • Proof of Hawaii residency (Hawaii ID, state driver's license, or mail to a Hawaii address)
  • Bank statements from recent months (checking, savings, CDs)

Med-QUEST processes applications within 45 days. After approval, it takes 60 to 90 days for SSA to stop deducting the Part B premium — but over-deducted premiums get refunded.
