Hawaii QMB Program: Eligibility, Application, and What It Covers
What QMB Covers in Hawaii
QMB — Qualified Medicare Beneficiary — is the most comprehensive tier of Hawaii's Medicare Savings Programs. If your parent qualifies, Med-QUEST pays:
- The monthly Part B premium ($202.90 in 2026)
- The Part A premium, if they don't get premium-free Part A
- The Part A inpatient hospital deductible ($1,736 per benefit period)
- The Part B annual deductible ($283)
- All Medicare copays and coinsurance
That last point matters more than most people realize. QMB doesn't just cover premiums — it eliminates cost-sharing on every Medicare-covered service. Providers who accept Medicare are legally barred from billing QMB enrollees for deductibles, copays, or coinsurance. If a provider sends a bill anyway, that's a violation of federal law, and there are steps to stop it.
QMB Income and Asset Limits in Hawaii (2026)
Hawaii uses higher Federal Poverty Level guidelines than the mainland. The 2026 QMB limits, with the $20 monthly income disregard already included:
| Individual | Couple | |
|---|---|---|
| Monthly income limit | $1,530 | $2,075 |
| Resource limit | $9,950 | $14,910 |
Med-QUEST counts gross monthly income — Social Security before any deductions, pensions, VA benefits, and annuities. An adult child's income doesn't count, even if the parent lives with them.
The resource limit exempts the home (fully exempt if occupied or with intent to return), one car, household goods, burial plots, and life insurance policies under $1,500 face value.
How to Apply for QMB in Hawaii
QMB applications go through Med-QUEST, not Social Security. You'll need:
- Medicare card showing Part A and/or Part B enrollment
- Social Security benefit letter with the gross monthly amount
- Proof of Hawaii residency (driver's license, state ID, or mail to a Hawaii address)
- Bank statements from recent months (checking, savings, CDs)
Submit through any of three channels:
- Online: KOLEA portal at medical.mybenefits.hawaii.gov
- Phone: 1-800-316-8005
- Paper: Form DHS 1100 plus supplemental DHS 1100B, mailed or faxed to your island's Med-QUEST eligibility office
Med-QUEST has 45 days to process the application. QMB coverage starts on the first day of the month after approval — there's no retroactive coverage for QMB, unlike SLMB and QI.

The Part B Premium Lag
After QMB approval, it takes 60 to 90 days for Social Security to stop deducting the Part B premium from your parent's check. During that window, SSA keeps withholding $202.90/month. Once the system updates, SSA refunds every over-deducted premium back to the QMB effective date.
If your parent gets a direct bill from CMS for Medicare premiums while the application is pending, they should keep paying if possible to avoid technical disenrollment. Those payments also get refunded once buy-in completes.
QMB for Seniors Without Premium-Free Part A
Hawaii is a Part A buy-in state. If your parent doesn't have 40 quarters of work history and would normally owe a Part A premium ($278 or $505/month in 2026), QMB can cover that premium — and they can enroll in Part A at any time of year through SSA's Conditional Enrollment process, without waiting for the General Enrollment Period.
The process: visit an SSA office and request Conditional Part A Enrollment under POMS HI 00801.140. SSA logs the enrollment but doesn't bill the applicant. Then apply for QMB through Med-QUEST. If approved, Hawaii pays the Part A premium. If denied, the conditional enrollment voids automatically — no bill.
QMB Automatically Triggers Extra Help
Once Med-QUEST approves QMB, your parent automatically qualifies for Extra Help (the Part D Low-Income Subsidy). No separate application needed. Extra Help caps prescription copays at $5.10 for generics and $12.65 for brand-name drugs, and eliminates the Part D deductible.
The Hawaii Medicare Savings Programs & Extra Help Guide walks through the full QMB application with Hawaii-specific income worksheets, the document checklist, and templates for handling billing disputes if a provider bills your parent after QMB approval.
