Hawaii Medicare Savings Program: What It Covers and How to Apply

What Hawaii's Medicare Savings Programs Actually Do

Medicare Savings Programs in Hawaii pay some or all of your Medicare costs — premiums, deductibles, and copays — depending on which tier you qualify for. The programs are run by the Department of Human Services through its Med-QUEST Division, not by Medicare or Social Security directly.

There are four tiers, and the one you land in determines what gets paid:

  • QMB (Qualified Medicare Beneficiary): Pays your Part A premium (if you owe one), Part B premium ($202.90/month in 2026), Part A and B deductibles, and all Medicare copays and coinsurance. This is the most comprehensive tier.
  • SLMB (Specified Low-Income Medicare Beneficiary): Pays your Part B premium only.
  • QI (Qualifying Individual): Also pays your Part B premium only, but uses a higher income cutoff than SLMB. Funding is capped federally and awarded first-come, first-served. You must reapply every year.
  • QDWI (Qualified Disabled Working Individual): Pays the Part A premium for disabled workers under 65 who lost premium-free Part A because they returned to work.

For most families, the immediate dollar impact is the Part B premium — $202.90/month, or $2,434.80/year — coming back into a parent's Social Security check.

Why Hawaii's Income Limits Are Higher Than the Mainland

Hawaii uses its own Federal Poverty Level guidelines, set roughly 15% above the 48 contiguous states. That means the income cutoffs for every MSP tier are higher here than the numbers you'll find on Medicare.gov or most national guides.

For a single individual in 2026 (with the standard $20 income disregard already built in):

Program Monthly Income Limit
QMB $1,530
SLMB $1,836
QI $2,066
QDWI $3,060

Resource limits for QMB, SLMB, and QI are $9,950 for an individual and $14,910 for a couple. Your home, one car, household goods, and burial plots are all exempt.

If your parent's gross Social Security is $1,500/month, they'd qualify for QMB. At $1,750/month, they'd qualify for SLMB. National guides showing a $1,350 QMB limit for the lower 48 don't apply here.

How MSP Enrollment Triggers Extra Help Automatically

Getting approved for any Medicare Savings Program — QMB, SLMB, or QI — automatically qualifies your parent for Extra Help (the Part D Low-Income Subsidy). Extra Help caps prescription copays at $5.10 for generics and $12.65 for brand-name drugs in 2026, and eliminates the Part D deductible entirely.

You don't need to apply for Extra Help separately once MSP approval goes through. CMS handles the enrollment automatically.

How to Apply Through Med-QUEST

Applications go to Med-QUEST, not to Social Security or Medicare. Three channels:

  1. Online: Through the KOLEA portal at medical.mybenefits.hawaii.gov
  2. Phone: Call 1-800-316-8005 (Oahu local: 808-524-3370)
  3. Paper: Submit Form DHS 1100 plus the supplemental DHS 1100B to your island's Med-QUEST eligibility office

You'll need your Medicare card, proof of Hawaii residency, Social Security benefit letter showing gross monthly income, and recent bank statements.

Med-QUEST must process your application within 45 days. Once approved, the timing depends on which tier: QMB coverage starts the first day of the month after approval (no retroactive coverage), while SLMB and QI can cover up to three months before you applied.

After approval, it takes 60 to 90 days for SSA to stop deducting the Part B premium from your parent's Social Security check. Any premiums deducted during that lag get refunded.

What Happens Next

The Hawaii SHIP program (State Health Insurance Assistance Program) offers free counseling to help with the application — call 808-586-7299 on Oahu or 1-888-875-9229 from neighbor islands. They screen for eligibility and answer questions but don't assemble application packets.

If you want the full walkthrough — income worksheets with Hawaii's FPL math, a document checklist, the complete island-by-island office directory, and templates for billing disputes and appeals — the Hawaii Medicare Savings Programs & Extra Help Guide covers the entire process from eligibility check through renewal.
