Hawaii Medicare Part A Buy-In: How QMB Covers the Premium

The Part A Premium Problem

Most people get Medicare Part A for free because they or a spouse paid into Social Security for at least 40 quarters (10 years). But seniors who immigrated to Hawaii later in life, worked in non-covered employment, or simply didn't accumulate enough work credits face a Part A premium of $278/month (30–39 quarters) or $505/month (fewer than 30 quarters) in 2026.

That's up to $6,060/year just for hospital coverage — on top of the $202.90/month Part B premium. For a low-income senior in Hawaii, these combined costs can consume a quarter or more of their monthly income.

Hawaii Is a Part A Buy-In State

Hawaii is classified as a Part A buy-in state under Section 1818(g) of the Social Security Act. This gives Hawaii seniors a major advantage over residents of "Group Payer" states.

The practical difference: in a buy-in state, a senior who qualifies for QMB can enroll in Medicare Part A at any time of year through SSA's Conditional Enrollment process. They don't have to wait for the General Enrollment Period (January through March) and don't face late enrollment penalties.

In a Group Payer state, that same senior would have to wait for the GEP, potentially going months without Part A coverage while exposed to full hospital costs.

How Conditional Part A Enrollment Works

The process has two steps — one at SSA, one at Med-QUEST:

Step 1: SSA Conditional Application. Your parent visits a Social Security office and requests Conditional Part A Enrollment, citing POMS HI 00801.140. The SSA technician records that the applicant wants Part A only if the state approves QMB. This generates an enrollment record but no premium bill.

Step 2: Med-QUEST QMB Application. With the SSA conditional enrollment receipt in hand, apply for QMB through Med-QUEST (online at medical.mybenefits.hawaii.gov, by phone at 1-800-316-8005, or by paper form DHS 1100 plus DHS 1100B to your island's eligibility office).

If QMB is approved: Med-QUEST transmits buy-in codes to CMS. Hawaii starts paying the Part A premium. Part A entitlement begins concurrent with the QMB effective date (the first day of the month after approval). Your parent now has Part A coverage with no premium.

If QMB is denied: The conditional enrollment automatically voids. No premium bill is ever generated. Your parent isn't stuck owing $278 or $505/month for Part A they can't afford.

QMB Income Limits for Part A Buy-In

The same QMB eligibility rules apply whether your parent already has Part A or is enrolling through the buy-in:

  • Individual: gross monthly income up to $1,530, resources up to $9,950
  • Couple: gross monthly income up to $2,075, resources up to $14,910

These are Hawaii's 2026 limits, which sit roughly 15% higher than the mainland because Hawaii uses its own Federal Poverty Level guidelines.

Why This Matters for COFA Citizens

Hawaii has a substantial population of Compact of Free Association (COFA) citizens from the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau. COFA citizens residing in Hawaii are fully eligible for Med-QUEST and Medicare Savings Programs without a five-year waiting period.

For COFA citizens who may not have U.S. work history, the Part A buy-in through QMB provides a pathway to hospital coverage that would otherwise be inaccessible.

What QMB Covers Beyond Part A

QMB doesn't just pay the Part A premium. It covers:

  • Part B premium ($202.90/month)
  • Part A deductible ($1,736 per benefit period)
  • Part B deductible ($283/year)
  • All Medicare copays and coinsurance
  • Automatic enrollment in Extra Help for Part D prescriptions

For a senior who would otherwise owe $505/month for Part A plus $202.90/month for Part B — $707.90/month total — QMB eliminates the entire cost and adds protection against hospital and doctor bills.

The Hawaii Medicare Savings Programs & Extra Help Guide walks through the Conditional Part A Enrollment process step by step, including exactly what to say at the SSA office and how to coordinate the timeline with the Med-QUEST application.
