If your parent is approved for Medicaid LTSS and receives home-based services, Rhode Island's Personal Choice Program allows you to be hired as their paid caregiver. What the Budget Covers
The monthly care budget is determined by EOHHS based on the clinical level-of-care assessment and the Person-Centered Plan developed with the conflict-free case manager. The budget reflects the number of authorized care hours and the approved service categories.
Budgets cover personal care assistance (bathing, dressing, toileting, transferring), homemaker services (meal preparation, cleaning, laundry), and in some cases companion services and transportation. The participant allocates the budget across these categories according to their needs.
The caregiver wage is set by the participant within the budget. Rhode Island has transitioned its self-directed programs from the old Independent Provider model (which set a flat wage of $13.82 per hour) to the unified Personal Choice Program. Who Qualifies as a Caregiver
Any adult who passes a background check can serve as a paid caregiver under Personal Choice. Family members are explicitly permitted with the exception of spouses, legal guardians, and the designated representative who manages the budget. The caregiver does not need professional licensure, CNA certification, or prior home care experience.
The caregiver must complete an orientation to the program and undergo a criminal background check. If the caregiver will be providing personal care (as opposed to homemaker services only), a tuberculosis screening may also be required depending on the managed care organization's policies.
The Enrollment Process
Getting into the Personal Choice Program follows a sequence:
- Your parent must be approved for Medicaid LTSS with a "high" or "highest" level-of-care determination
- During the Person-Centered Planning meeting with the CFCM case manager, the family requests self-directed care under the Personal Choice Program
- EOHHS authorizes a monthly care budget based on the assessed needs
- The participant (or representative) selects a fiscal intermediary
- The participant hires their caregiver, and the caregiver completes the background check and orientation
