Every Australian state publishes free enduring power of attorney forms. The forms themselves cost nothing. Where to Download Free EPOA Forms by State
Each state and territory has its own form, and using the wrong one makes the document void in that jurisdiction:
- NSW — Enduring Power of Attorney form from the NSW Land Registry Services site, plus a separate Appointment of Enduring Guardian for personal and medical decisions.
- Victoria — The EPOA form under the Powers of Attorney Act 2014. Victoria also requires a separate Appointment of Medical Treatment Decision Maker.
- Queensland — Short Form 2 or Long Form 3 under the Powers of Attorney Act 1998. QLD's EPOA covers both financial and personal/health decisions in a single document.
- Western Australia — The EPA form from the WA Public Trustee site, plus a separate Enduring Power of Guardianship for lifestyle and medical decisions.
- South Australia — Financial EPOA from the SA Government. Personal and healthcare decisions require a separate Advance Care Directive under the Advance Care Directives Act 2013.
- Tasmania — Form 3 or Form 4 under the Powers of Attorney Act 1934. Tasmania is the only state where the EPOA must be registered with the Land Titles Office before it has any legal effect.
- ACT — The single EPOA form under the Powers of Attorney Act 2006, which can cover financial, personal, health, and medical research decisions.
- Northern Territory — The Advance Personal Plan under the Advance Personal Planning Act 2013, which combines financial, personal, and healthcare decision-making into one document.
There is no national EPOA form. A generic template downloaded from a commercial site that doesn't match your state's specific requirements will be rejected.
What Does an EPOA Actually Cost?
The form is free. The real costs sit elsewhere:

Can You Set Up an EPOA Without a Lawyer?
Yes — every state's forms are designed for self-completion. But "designed for self-completion" doesn't mean "impossible to get wrong." The most common DIY failures:
Witnessing errors. Each state has strict rules about who can witness the signing, and getting this wrong voids the document entirely. In Victoria, you need two adult witnesses and one must be a medical practitioner or authorised affidavit witness. In NSW, a prescribed witness (solicitor, barrister, or registrar) must sign a certificate confirming the principal understood the document. In Queensland, you need a JP, Commissioner for Declarations, solicitor, or notary public. Relatives, the appointed attorneys, and paid carers are disqualified from witnessing in most states.
Missing the second document. In NSW, WA, and Tasmania, financial and personal/medical decisions require completely separate legal instruments. Setting up only a financial EPOA leaves nobody authorised to consent to surgery, choose an aged-care facility, or make accommodation decisions.
Not checking the property-registration rule. Except in Tasmania (where registration is mandatory for all purposes), you don't need to register an EPOA for everyday banking. For a property transaction, the requirement varies: Victoria does not register standard EPOAs, WA describes registration as optional but highly recommended, and the other jurisdictions have their own land-title requirements.
No attorney acceptance. An EPOA isn't effective until each appointed attorney signs a formal Statement of Acceptance. The principal and witness must sign on the same day in each other's presence, but attorneys can sign their acceptance later. Until they do, they have no legal authority.

Online EPOA Platforms: What They Do and Don't Cover
Commercial platforms like Willed and similar services charge $50–$200 to generate a state-specific EPOA through an online questionnaire. They handle the form-filling, but they don't attend the signing appointment, check the witnesses' eligibility, or lodge the document with land registries.

The hard part is organising the correct witnesses, getting a GP capacity assessment if there's any doubt, and making sure the document is registered and lodged with every institution that will need it — the bank, My Aged Care, Centrelink, and potentially the Land Titles Office.
