DC Senior Health Benefits for Fixed-Income Residents — Programs That Actually Help

A DC senior living on $2,200 a month in Social Security and a small pension is spending more than $200 of it on Medicare Part B before they fill a single prescription or see a doctor. Most assume that's just how it works — the premium gets deducted, the budget gets tighter, and they figure out the rest.

The District has programs that eliminate most of those costs. The problem isn't availability — it's that nobody connects the dots between programs that are run by different agencies under different names.

The Biggest One: DC's Medicare Savings Program (QMB)

This is the single most impactful program for seniors on fixed incomes, and the one most frequently missed. DC's QMB program pays:

  • The monthly Part B premium ($202.90 in 2026)
  • Part A premium for seniors without 40 work quarters (up to $565/month)
  • All Medicare deductibles and coinsurance
  • Automatic Full Extra Help on prescriptions (caps drug copays at $5.10/$12.65)

DC expanded QMB eligibility to 300% FPL — $4,010/month for an individual, $5,430 for a couple. There's no asset test. A senior with $80,000 in savings earning $3,500/month qualifies. The standard 2026 federal QMB income limit is $1,350/month for an individual, with a $9,950 resource limit.

Apply through District Direct, at any ESA service center, or by phone at (202) 727-5355.

Full Medicaid (Aged, Blind, or Disabled)

For seniors with lower incomes (roughly under $1,715/month with DC's local disregards) and limited assets ($4,000 individual), full ABD Medicaid adds services QMB doesn't cover: adult dental, vision, personal care aides, non-emergency transportation, and medical equipment. Seniors can have both QMB and full Medicaid simultaneously.

DC SHIP — Free Medicare Counseling

The State Health Insurance Assistance Program, hosted by the Department of Aging and Community Living, provides free one-on-one Medicare counseling. SHIP counselors help with Part D plan comparisons, Medigap questions, and benefits applications — without selling anything.

Department of Aging and Community Living (DACL)

DACL runs programs beyond SHIP that serve DC seniors:

  • Senior wellness centers across the District
  • Home-delivered meals and congregate nutrition
  • Transportation assistance
  • Caregiver support services
  • Information and referral through the DC Aging and Disability Resource Center

Legal Aid for Benefits Problems

When applications get denied, renewals get lost, or providers send bills they shouldn't, several organizations provide free legal help:

  • Legal Counsel for the Elderly (AARP affiliate): (202) 434-2120
  • Bread for the City Legal Clinic: (202) 265-2400 (NW) / (202) 561-8587 (SE)
  • Legal Aid Society of DC: (202) 628-1161
  • Neighborhood Legal Services Program: (202) 832-6577

These organizations prioritize acute legal crises (evictions, benefit terminations), but they handle QMB and Medicaid disputes regularly.

Prescription Drug Help

DC doesn't run a separate state pharmacy assistance program — it doesn't need one. QMB automatically triggers Full Extra Help, which eliminates the Part D deductible, covers premiums on benchmark plans, and caps copays. For DC seniors who qualify for QMB (many do), prescription costs drop dramatically without any additional application.

Community health centers like Unity Health Care, Bread for the City, and Mary's Center also offer 340B discounted prescriptions and sliding-scale primary care.

The DC Health Care Ombudsman

If your parent runs into a billing dispute, a coverage denial, or a provider that won't accept their QMB status, the DC Office of the Health Care Ombudsman and Bill of Rights handles complaints at no cost: 1-877-685-6391.

Where to Start

For most DC seniors on fixed incomes, QMB is the foundation. It has the highest income threshold, no asset test, and it triggers the most automatic downstream benefits. Start there, and layer on other programs as needed.

The DC Medicare Savings Programs & Extra Help Guide provides the complete eligibility calculation, application walkthrough, and post-approval steps for QMB, plus how it connects to every other program listed here.
