$0 Delaware — Medicare Savings Program Eligibility Checklist

Delaware Senior Benefits Programs for Low-Income Medicare Recipients

A Delaware senior living on Social Security alone faces a stack of healthcare costs that eat into an already tight budget — Part B premiums, prescription copays, deductibles, coinsurance. The state runs several programs that can eliminate most of these costs, but they're administered by different agencies and most families don't realize they can be combined.

Medicare Savings Programs (QMB, SLMB, QI)

These three tiers cover Medicare premiums and cost-sharing based on income. Delaware has eliminated the asset test for all three, so savings, home equity, and retirement accounts don't count.

  • QMB (income up to $1,350/month individual): Covers Part B premium, Part A premium if needed, all deductibles, coinsurance, and copays
  • SLMB (up to $1,616/month): Covers the Part B premium — $202.90/month back in the Social Security check
  • QI (up to $1,816/month): Also covers the Part B premium, under a capped federal allotment

Apply through Delaware ASSIST (assist.dhss.delaware.gov) or at any DSS State Service Center. One application screens for all three tiers.

Extra Help (Part D Low-Income Subsidy)

Extra Help reduces prescription drug costs under Medicare Part D — eliminating the deductible, capping copays at $5.10 for generics and $12.65 for brand-name drugs, and covering most of the monthly Part D premium.

Any MSP approval automatically triggers Extra Help with no separate application. For seniors above MSP income limits but below 150% FPL ($1,995/month individual), apply directly through SSA at ssa.gov/extrahelp. In 2026, applicants between 100% and 150% FPL must meet Extra Help resource limits of $18,090 for an individual or $36,100 for a couple, including the burial exclusion; applicants below 100% FPL can qualify for the full premium subsidy regardless of resources.

DPAP (Delaware Prescription Assistance Program)

Delaware's own drug assistance program provides up to $3,000 per year toward prescription costs and Part D premiums. Eligibility extends to 200% FPL ($2,660/month individual) — higher than MSP or Extra Help limits.

Part D is primary. DPAP can help with Part D premiums, deductibles, and eligible coverage-gap drug costs, but it does not pay Part D copayments. For a DPAP-covered prescription, the member pays 25% of the prescription cost or $5, whichever is greater; assistance toward covered Part D drug costs counts toward the $2,100 annual out-of-pocket cap.

The catch: if eligible for Medicare, a DPAP recipient must show proof of Part D enrollment and Extra Help enrollment, if eligible, within 90 days from the date DPAP benefits begin. It's designed to layer on top of federal benefits, not replace them.

How They Stack Together

The most valuable combination for a Delaware senior below 100% FPL:

  1. QMB eliminates Part B premiums, deductibles, and all Medicare cost-sharing
  2. Extra Help (automatic with QMB) slashes Part D prescription costs to minimal copays
  3. DPAP provides an additional $3,000 annual benefit for remaining drug costs and Part D premiums
  4. The 2026 Part D out-of-pocket cap ($2,100) means total annual prescription costs are bounded, with DPAP payments counting toward that cap

A senior at 130% FPL who doesn't qualify for QMB or SLMB still gets QI (Part B premium covered), and can separately qualify for Extra Help (under 150% FPL) and DPAP (under 200% FPL). The programs don't cancel each other out — they layer.

Free Counseling: Where to Start

DMAB (Delaware Medicare Assistance Bureau): 1-800-336-9500. Delaware's official SHIP program, formerly known as ELDERinfo. Free, unbiased counselors help with program screening, Part D plan comparisons, and application guidance. Located in the Department of Insurance, not connected to any insurance company.

Delaware ADRC (Aging and Disability Resource Center): 1-800-223-9074. Broader aging services including connections to home care, caregiver support, meal programs, and transportation. Not Medicare-specific, but a good starting point when multiple needs overlap.
