DC Medicare Savings Program Renewal — Deadlines, Documents, and the 90-Day Grace Period

DC's QMB coverage doesn't last forever. It's certified for 12 months, and your parent needs to renew it — or risk losing the Part B premium payment, the cost-sharing protection, and the automatic Extra Help that comes with it.

The good news: DC has several built-in protections that make renewal easier than the initial application, including an automated process that may renew coverage without any paperwork at all.

The Renewal Timeline

Ninety days before the 12-month certification expires, ESA mails a Non-MAGI Renewal Packet to your parent's address on file. This is the official signal that recertification is due.

Your parent can submit the renewal through any of the same channels used for the initial application:

  • Online through District Direct (districtdirect.dc.gov)
  • Through the District Direct mobile app
  • By mail to the Case Record Management Unit (P.O. Box 91560, Washington, DC 20090)
  • By fax to (202) 671-4400
  • In person at any ESA Service Center

Ex Parte Renewal — The Automatic Path

ESA also attempts an automated ex parte renewal. This means the agency cross-references electronic databases — primarily SSA benefit feeds — to verify that your parent's income still falls within the limit.

If the data confirms ongoing eligibility, coverage renews automatically. Your parent doesn't need to do anything. ESA sends a notice confirming the renewal.

If the electronic check can't verify eligibility (for example, because of changed income or a data mismatch), your parent must respond to the renewal packet.

What the Renewal Packet Requires

The renewal is simpler than the initial application. ESA already has your parent's identity documents and Medicare enrollment on file. The renewal typically focuses on:

  • Current monthly income — updated SSA-1099 or pension stubs if income changed
  • Address confirmation — critical, since a wrong address means renewal packets go to the wrong place
  • Household composition changes — marriage, separation, new household members

If nothing has changed since the last certification, the renewal packet is short. Fill it out, sign it, and submit.

What Happens If Your Parent Misses the Deadline

If the renewal packet isn't submitted by the certification end date, ESA terminates coverage. Your parent stops getting the Part B premium paid by the District, and Medicare cost-sharing protections end.

But DC provides a safety net: a 90-day grace period after termination. If your parent submits the renewal form within those 90 days and still qualifies, ESA reinstates QMB coverage retroactively to the certification end date. There's no gap in premium buy-in payments.

After 90 days, the grace period expires. Your parent would need to file a completely new application from scratch.

Common Renewal Problems

Missed the renewal notice: If your parent moved or if District Direct's messaging system failed to deliver the notice, they may not know renewal is due. Set a calendar reminder for 30 days before the anniversary of the original QMB approval date.

Income increased above the limit: If your parent's countable income exceeds $4,010/month (individual) or $5,430/month (couple) at renewal, they won't qualify. ESA will send a termination notice explaining the reason and the appeal rights.

Address not updated: ESA mails renewal packets to the address on file. If your parent moved within DC without notifying ESA, the packet goes to the old address. Report address changes to ESA within 10 days of moving — through District Direct, by phone at (202) 727-5355, or in person.

The Renewal Defense Kit

The DC Medicare Savings Programs & Extra Help Guide includes a renewal calendar that tracks certification dates, the complete list of documents needed for renewal, and instructions for recovering within the 90-day grace period if a deadline gets missed.
