Colorado Authorized Representative PEAK Medicare
Managing a parent's Medicare Savings Program application from out of state — or even from across town — requires more than just logging into their PEAK account. Without formal authorized representative status, the county can't discuss the case with you, and every notice goes only to your parent's address.
The Difference Between Helping and Having Authority
Anyone can sit next to a parent and help them fill out the PEAK application. You can type the information, upload the documents, and hit submit. But signing as a "preparer" on the application gives you zero legal authority over the case.
Without authorized representative status:
- The county won't discuss eligibility details, pending documents, or case status with you
- All Notices of Action and Requests for Information go only to the applicant
- You can't complete renewals, respond to verification requests, or file appeals on their behalf
- You can't receive copies of official correspondence
For a parent with cognitive decline, limited English, or difficulty managing mail, this gap can be fatal to the application. A Request for Information arrives, sits unopened for 12 days, and the response window may be nearly over, depending on the deadline in the notice.
Setting Up Authorized Representative Status
Your parent and you must complete HCPF's Authorized Representative Designation Form. This form:
- Names you as the designated representative
- Grants authority to receive copies of all Notices of Action
- Allows you to complete renewals and respond to verification requests
- Authorizes you to represent the applicant at administrative fair hearings
Submit the signed form to the county Department of Human Services along with the application, or upload it through PEAK. Once it's processed, you should start receiving copies of all official correspondence.
Managing PEAK Remotely
With authorized representative status established, you can manage the entire process through PEAK from anywhere:
- Submit the initial application
- Upload verification documents (Medicare card, Social Security Award Letter, bank statements)
- Track application status
- Complete annual renewals
- Respond to Requests for Information within the deadline
The PEAK Health mobile app also allows document uploads from a phone — useful for photographing documents your parent has at their home and uploading them immediately.

The SSA Representation Gap
Here's the piece that catches every out-of-state caregiver off guard: Colorado's HCPF Authorized Representative form has no authority at the Social Security Administration. They're separate systems, separate agencies, separate paperwork.
If you need to discuss your parent's Medicare enrollment, Part B premium, Extra Help status, or buy-in processing with SSA, you need SSA's own authorization. That means either:
- Form SSA-1696 (Appointment of Representative) — specifically for representing someone in SSA matters
- Power of Attorney — a legal document recognized by SSA, though they may request their own forms as well
Without one of these, SSA won't tell you whether the state buy-in has been received, when the Part B deduction will stop, or whether Extra Help was activated. You'll know something changed only when the Social Security deposit amount changes.
When Power of Attorney Isn't Enough
A general Power of Attorney covers financial management but may not be accepted by all agencies without their own supplementary forms. HCPF accepts the state Authorized Representative Form. SSA prefers its own SSA-1696. Some county offices will also accept a Colorado Statutory Power of Attorney, but processing can be slower.
The most reliable approach: file the HCPF Authorized Representative form for the state side and SSA-1696 for the federal side. Two forms, complete coverage across both agencies.
Our Colorado Medicare Savings Programs & Extra Help Guide includes the step-by-step representative setup process for both agencies and the communication log template for tracking every interaction with county workers and SSA.
