How to Apply for Kentucky Medicare Savings Programs as a Caregiver or Authorized Representative
When Your Parent Can't Navigate DCBS Alone
The typical Kentucky Medicare Savings Program applicant is a senior on a fixed Social Security check who finds government paperwork overwhelming. The typical person who actually completes the application is their adult child — often from another city or another state entirely.
DCBS recognizes this reality. Kentucky has a formal authorized representative process that lets a caregiver apply, submit documents, receive official notices, and manage the kynect account on behalf of an aging parent. But the process requires a specific form, and skipping it creates gaps that delay or derail the application.
Filing Form MAP-14: Interested Party Authorization
Form MAP-14 is the Interested Party Authorization Form used by the Cabinet for Health and Family Services. It grants you authority to:
- Complete and submit the MSP application (Form MAP-205 or the kynect online application) on your parent's behalf
- Submit income and asset verification documents
- Receive copies of official CHFS/DCBS notices, including approval letters, denial notices, and renewal packets
- Communicate with DCBS caseworkers about the case
Your parent signs the MAP-14 authorizing you. Submit it alongside the MAP-205 application — or beforehand if you want to get the authorization on file before starting the process. It can be mailed to the local county DCBS office, faxed to the centralized number at 502-573-2005, or submitted through the kynect portal.
If your parent has an existing Power of Attorney or court-ordered guardianship, those documents also work in place of the MAP-14. But the MAP-14 is lighter — it doesn't require a lawyer and it's specific to CHFS benefits.
Managing the kynect Account Remotely
The kynect.ky.gov portal handles MSP applications, document uploads, and status tracking. As an authorized representative, you can create and manage your parent's kynect account from anywhere with an internet connection.
A few practical notes for remote caregivers:
Set up the account using your parent's information — name, Social Security number, date of birth, Kentucky mailing address. The account belongs to the beneficiary, not the representative.
Use your email address for notifications if your parent doesn't check email. kynect sends status updates and document requests to the account's email. A renewal notice that sits unread in an inbox your parent never opens is functionally the same as a lost letter.
Upload documents digitally. Photos of bank statements, the Social Security Award Letter, and the Medicare card taken with a phone camera are accepted. Scanned PDFs work too. This eliminates the need to mail originals from another state and wait for DCBS to receive them.
Check the portal regularly during the 20-to-40-day processing window. If DCBS needs additional documentation, the request appears on kynect. Responding quickly prevents the verification window from expiring.

When You're Out of State
Long-distance caregiving adds a specific challenge: you can't walk into a county DCBS office when something goes wrong. The phone and digital channels become critical.
DCBS Family Support line: 1-855-306-8959. With your MAP-14 on file, you can discuss your parent's case and get status updates.
Kentucky SHIP: 1-877-293-7447. Free, unbiased Medicare counseling.
kynect support line: 1-855-459-6328. For portal issues — locked accounts, document upload problems, password resets.
Keep copies of everything you submit. Screenshot the kynect confirmation pages. Note the date and time of every phone call with the representative's name. If a renewal notice gets lost or a document goes missing, your records prove what was submitted and when.
What You Can't Do as a Representative
MAP-14 authorizes you to complete applications and submit verifications. For who must sign the MAP-205, follow the form instructions or ask DCBS. If your parent cannot sign, a valid Power of Attorney or court-ordered guardianship document can also be submitted.
The Kentucky Medicare Savings Programs & Extra Help Guide includes the full authorized representative setup process, a document submission checklist, and an agency communication log designed for caregivers managing the application from a distance.
