Alaska Part A Buy-In: How to Get Medicare Part A Through QMB Year-Round
Seniors who did not work 40 quarters of Medicare-covered employment do not get premium-free Part A. The standard Part A premium runs over $500 a month in 2026, and in most states, enrolling requires waiting for the General Enrollment Period from January through March. Alaska is different.
Alaska Is a Part A Buy-In State
Under the Social Security Administration's classification system (POMS HI 00801.140), Alaska holds a Category 3 State Buy-In Agreement with CMS. This means the state's master buy-in agreement automatically covers Part A premium payments for QMB enrollees.
The practical consequence: an Alaskan who lacks premium-free Part A can file a conditional Part A enrollment at any SSA field office, at any time of year. There is no need to wait for the January-through-March General Enrollment Period. There is no late enrollment penalty. And the applicant does not receive a direct bill for the Part A premium.
How the Conditional Enrollment Works
- The beneficiary (or their representative) visits an SSA field office in Alaska and requests conditional enrollment in Premium Part A
- SSA takes the application and notes in the remarks field that Part A enrollment is contingent on QMB approval by Alaska DPA
- The beneficiary applies for QMB through DPA (if they have not already)
- Once DPA approves the QMB application, the state transmits a buy-in accretion record to CMS
- The Part A premium is paid by the state — the beneficiary never sees a bill
The conditional enrollment does not generate a premium invoice to the applicant. It sits in pending status until DPA's QMB determination comes through. If QMB is approved, the state picks up the premium. If QMB is denied, Alaska will not assume the Part A premium through the buy-in; contact SSA about the next step before pursuing regular enrollment.
When SSA Staff Push Back
Some SSA field office staff are not familiar with the Part A buy-in classification and may insist that an applicant must wait for the General Enrollment Period. This happens because the buy-in rules vary by state and are documented in SSA's internal POMS system, not in the public-facing Medicare materials.
If this happens, reference the following:
"Per SSA POMS HI 00801.140 Section C and POMS HI 00815.030, Alaska is a Part A Buy-In State. Under these guidelines, the applicant is authorized to file a conditional Part A enrollment application outside the General Enrollment Period without incurring personal premium liability, pending Alaska DPA's QMB determination."
Ask the SSA employee to check the POMS citation. This is internal SSA policy that the field office has access to.

Who Benefits Most
This matters most for:
- Immigrants who arrived after age 50 and will never accumulate 40 quarters of covered employment
- Federal or state retirees whose employment was not covered by Social Security or Medicare
- Spouses who relied on a partner's work history but do not qualify for premium-free Part A on their own
- Workers with fewer than 30 quarters who face the full Part A premium (over $500/month in 2026)
Without the conditional enrollment path, these individuals would generally have to wait for the next GEP to enroll in premium Part A.
The QMB Connection
Conditional Part A enrollment only works because QMB covers the Part A premium. SLMB and QI cover only the Part B premium and do not trigger Part A buy-in. If the applicant's income qualifies for QMB (under $1,683/month single in Alaska), the conditional path is available. If income is too high for QMB but qualifies for SLMB or QI, the Part A premium is not covered.
The Alaska Medicare Savings Programs & Extra Help Guide includes the exact POMS citations, a checklist for the SSA field office visit, and a step-by-step process for coordinating the conditional Part A enrollment with the DPA QMB application.
