Maine Part A Buy-In — Medicare With No Work Credits

The Problem: No Work History, No Free Part A

Medicare Part A is generally premium-free when you or an eligible spouse have 40 or more quarters of Medicare-covered employment. Without enough qualifying work credits on either record, the Part A premium runs up to $565 per month in 2026 — a cost that puts hospital coverage out of reach for people who spent careers as homemakers, in non-covered government work, or outside the US workforce.

Normally, someone without premium-free Part A can only enroll during the General Enrollment Period (January 1 through March 31), with coverage starting July 1. Miss that window and you wait another year.

Maine changes both of those problems.

Maine Is a Part A Buy-In State

Under Section 1818(g) of the Social Security Act, Maine is classified as a Part A buy-in state. This designation creates a year-round pathway to Part A through the QMB program, bypassing both the premium and the enrollment period restriction.

Here's how it works, step by step:

Step 1: The applicant visits an SSA field office and requests conditional Part A enrollment under POMS HI 00801.140, applying for Part B too if not already enrolled. This can be done at any time of year — no need to wait for the General Enrollment Period.

Step 2: SSA creates a conditional Part A entitlement record and does not bill the Part A premium while the state decides the QMB application. If the applicant also applies for Part B, that enrollment is not conditional and Part B premiums remain the applicant's responsibility.

Step 3: The applicant takes the SSA conditional receipt to Maine DHHS OFI and files a QMB application.

Step 4A — If QMB is approved: MaineCare transmits a Part A buy-in code to CMS. The state pays the applicable Part A premium (up to $565/month in 2026) and the Part B premium ($202.90/month). The applicant owes nothing.

Step 4B — If QMB is denied: SSA automatically voids the conditional Part A enrollment. The applicant owes no Part A premium under that conditional enrollment. Any Part B coverage remains in effect and its premiums remain the applicant's responsibility.

The Conditional Part A Protection

This is the key feature: conditional Part A enrollment creates no Part A premium liability while OFI decides the QMB application. If QMB is approved, the state pays the Part A premium. If QMB is denied, SSA voids the conditional Part A enrollment. If the applicant also enrolls in Part B, that enrollment is not conditional, and Part B premiums remain the beneficiary's responsibility until QMB begins.

Late enrollment penalties are also waived when Part A is obtained through the buy-in. Someone who delayed enrollment for years doesn't face the permanent premium surcharges that normally apply.

SSA Staff May Not Know This Procedure

POMS HI 00801.140 is a specialized provision that not all SSA field office personnel handle regularly. If the representative at the window isn't familiar with conditional Part A enrollment in a buy-in state, cite POMS HI 00801.140 Section B.3 ("Part A Buy-In State Policy") and ask for a supervisor.

The process is established federal policy — it's not a workaround or special favor. But because it's uncommon, persistence at the SSA office sometimes matters.

Who Benefits Most

This pathway is most valuable for:

  • Immigrants who worked primarily outside the US and lack 40 quarters
  • Homemakers whose spouses are deceased or from whom they're divorced (and the marriage lasted under 10 years, disqualifying them from spousal benefits)
  • People who worked in non-covered government or railroad employment before the 1983 reforms
  • Anyone who simply didn't accumulate enough work history

For any of these situations, the conditional Part A plus QMB combination gives full hospital coverage at no cost to the beneficiary, available year-round.

The Maine Medicare Savings Programs guide walks through the SSA conversation, what documents to bring, and how to coordinate the conditional Part A filing with the QMB application timeline.
