Mississippi Part A Buy-In: QMB Conditional Enrollment Explained
The Problem: No Work Credits, No Free Part A
Most Medicare beneficiaries get Part A (hospital coverage) premium-free because they or their spouse earned at least 40 quarters of Medicare-covered employment. But some seniors — immigrants who arrived later in life, people who worked in non-covered employment, or those with fewer than 30 quarters — pay the full Part A premium of $565 per month in 2026. People with 30 to 39 quarters pay the reduced premium of $311 per month.
For a senior living on $1,200 a month in Social Security, that premium is impossible to absorb. Without Part A, they have no hospital insurance at all.
Mississippi's Part A Buy-In State Advantage
Mississippi is classified as a Part A Buy-In State under Section 1818(g) of the Social Security Act and SSA's POMS HI 00801.140. This gives low-income residents a specific pathway: if they qualify for QMB, the state pays the Part A premium on their behalf.
Three protections come with buy-in state classification that you won't find everywhere:
Year-round enrollment. Normally, you can sign up for premium Part A during the General Enrollment Period (January 1 – March 31 each year), with coverage starting the month after enrollment. In a Part A Buy-In State, someone not already enrolled in Part B can apply to SSA for Part B and conditional Part A enrollment any time of year. Someone already enrolled in Part B applies to DOM for QMB; if approved, the state can enroll them in Part A through the buy-in.
No Part A premium liability. The conditional enrollment means Part A activates only if DOM approves QMB. If the QMB application is denied, SSA does not enroll the applicant in Part A, so no Part A premium is due. Part B enrollment is not conditional; if the applicant enrolls in Part B as part of this process, coverage starts regardless of DOM's decision and they remain responsible for the Part B premium.
Late enrollment penalties waived. Under the state buy-in agreement, CMS waives all Part A late enrollment penalties for QMB enrollees. The premium stays at the standard rate regardless of how many years the person went without Part A.
How the Conditional Enrollment Process Works
For someone not already enrolled in Part B, use this conditional enrollment sequence. If the applicant already has Part B, skip the conditional SSA application and apply to DOM for QMB.
Step 1: Visit the local SSA office. If not already enrolled in Part B, the applicant files for Part B and conditional premium Part A. They must explicitly tell the SSA representative: "I am filing for conditional Part A enrollment under POMS HI 00801.140 in order to apply for the Mississippi QMB program."
Step 2: SSA adds mandatory remarks. The SSA representative records in the application remarks: "I wish to enroll in Part A only if my State approves my application for the Qualified Medicare Beneficiary (QMB) program."
Step 3: Apply at DOM. The applicant submits Form DOM-300 to their DOM regional office for QMB evaluation. If not already enrolled in Part B, include proof of the conditional Part A filing from SSA.
Step 4: DOM processes QMB and accrets Part A. If the applicant's total monthly income is less than $1,380 for an individual ($1,854 for a couple), and they meet the other QMB requirements, DOM approves QMB and sends an accretion file to CMS. Part A activates, and DOM pays the monthly premium directly to CMS.

Who This Applies To
Conditional Part A buy-in is primarily relevant for:
- Lawful Permanent Residents who arrived in the U.S. later in life and don't have 40 work quarters
- People who worked in non-covered employment (some government, religious, or nonprofit positions that didn't pay into Medicare)
- Disabled individuals under 65 who lost premium-free Part A after returning to substantial gainful work (though these individuals may also qualify for QDWI)
If your parent already has premium-free Part A, this mechanism isn't needed — they can apply directly to DOM for QMB without the SSA conditional step.
