Alabama Medicaid Coverage Terminated: How to Reapply for Medicare Savings
One missed renewal form and your QMB or SLMB benefits disappear — suddenly that $202.90 Part B premium is back in your Social Security deduction, and you're wondering whether you have to start the whole application process from scratch. Alabama has a 90-day window in which a renewal packet can reopen your case without a new application; after that, you can reapply.
The 90-Day Reopening Window
If your Medicare Savings Program coverage was terminated because you didn't return renewal paperwork on time, Alabama Medicaid allows a 90-day grace period from your termination date. During this window, submitting the completed renewal packet reopens your case without requiring a brand new Form 210 application.
The 90-day clock starts on the termination date printed in your closure notice, not the date you received it. If you're past 60 days, act immediately. After 90 days, submit a new Form 210.
Why Terminations Happen
Most MSP terminations in Alabama come down to administrative paperwork:
Unreturned renewal form. Alabama Medicaid sends Form 210-R (the renewal packet) annually. You have 30 days to complete, sign, and return it with current proof of income. If the form doesn't come back, the system closes your case automatically.
Address on file is wrong. If you moved and didn't update your address with Alabama Medicaid within the required 10 calendar days, the renewal packet went to your old address and the deadline passed without you knowing.
Income change pushed you over the limit. A Social Security cost-of-living adjustment or a new pension can push your gross income above the QMB threshold ($1,350/month individual in 2026). Alabama Medicaid terminates the current tier — but you may still qualify for SLMB ($1,616 limit) or QI-1 ($1,816 limit).
QI-1 federal funding cycle. QI-1 benefits are capped by federal block grant allocations and require annual re-application. Missing this cycle means waiting for the next funding period.
How to Reapply After the 90-Day Window
If the grace period has passed, you submit a new Form 210 to your regional Medicaid District Office or mail it to the Montgomery Eligibility Division. The process is identical to a first-time application:
- Complete Form 210 with current income information
- Attach your Medicare card copy, recent SSA benefit letter or SSA-1099, and photo ID
- Submit by mail (Certified Mail recommended), fax, in person, or phone (1-800-362-1504)
Alabama processes MSP applications within 45 calendar days.
Remember that Alabama has no asset test for Medicare Savings Programs. If the reason you hesitated to reapply was concern about savings or home equity, that's not a factor.

Getting Premiums Refunded After Reinstatement
If you lost QMB, SLMB, or QI-1 and had Part B premiums deducted from your Social Security check during the gap, here's what happens once you're re-approved:
For SLMB and QI-1, Alabama can provide up to 3 months of retroactive coverage prior to your new application month. If you reapply quickly, those gap-month premiums may be refunded.
For QMB, coverage starts the first day of the month after approval — no retroactive coverage. Premiums paid during the gap between termination and re-approval are not recoverable through the MSP program.
Once re-approved, allow 60–90 days for SSA to process the state buy-in record and stop deducting Part B premiums from your check. Any premiums deducted after your coverage effective date will be refunded as a lump sum.
Prevent It From Happening Again
Update your mailing address with Alabama Medicaid within 10 days of any move — this is a regulatory requirement, not just a suggestion. If you have an adult child or representative managing your benefits, make sure they're formally designated on file so they receive copies of renewal notices.
The Alabama Medicare Savings Programs & Extra Help Guide includes a renewal preparation checklist and an agency communication log for tracking every document submission, so nothing falls through the cracks at renewal time.
