Who Acentra Health Is and What They Do
Acentra Health is the federally designated Beneficiary and Family Centered Care Quality Improvement Organization (BFCC-QIO) for CMS Region 1, which covers New Hampshire along with the rest of New England. They're the independent body that reviews Medicare discharge disputes and quality-of-care complaints — separate from the hospital, separate from the insurer, and paid by CMS to provide an unbiased clinical review.
When a hospital or post-acute provider tries to end Medicare-covered care and you believe the decision is premature or unsafe, Acentra Health is who you call. They don't work for the hospital. They don't work for your parent's insurance plan. Their job is to review the clinical evidence and make a binding determination about whether continued care is medically necessary.

When to Call Acentra Health
There are two primary scenarios where contacting Acentra Health triggers a formal review:
Hospital discharge appeals. If your parent is a Medicare inpatient and the hospital is planning a discharge you believe is premature, call Acentra Health before your parent leaves the hospital and before midnight on the scheduled discharge date. This triggers what's known as a Weichardt review. The moment you file, the hospital is legally prohibited from discharging your parent or billing them for continued care while Acentra Health reviews the clinical record. Their independent physician reviewers typically issue a determination within 24 hours of receiving the hospital's complete documentation.
Post-acute service termination appeals. When a skilled nursing facility, home health agency, or hospice provider plans to end Medicare-covered services, the provider must issue a Notice of Medicare Non-Coverage (NOMNC) at least two calendar days before the termination date. You must contact Acentra Health before noon on the day before services are scheduled to end — a tighter deadline than the hospital appeal. For original Medicare beneficiaries, this is a BIPA review; for Medicare Advantage enrollees, it's a Grijalva review.
The Grijalva distinction matters: if your parent has Medicare Advantage and you miss the noon deadline, Acentra Health cannot review the case. The appeal goes to the managed care company instead, which doesn't provide the same independent protections.
What Happens During the Review
After you call, the hospital or provider must deliver a Detailed Notice explaining the clinical rationale for ending care. For hospital discharges, this is the Detailed Notice of Discharge (Form CMS-10066), due by noon the next day. For post-acute terminations, it's the Detailed Explanation of Non-Coverage (Form CMS-10124), due by end of day.
Acentra Health's physician reviewers then evaluate whether the clinical evidence supports continued Medicare coverage. They look at the patient's medical records, therapy notes, vital signs, functional assessments, and the provider's stated rationale for ending care.
You can submit additional information to strengthen your case. A written statement describing specific functional limitations, unresolved medical issues, or safety concerns at the proposed discharge destination can influence the review. If your parent's attending physician disagrees with the discharge decision, ask them to document their clinical opinion — that carries significant weight.

Beyond Discharge Appeals
