Wyoming Dual Eligible Medicare and Medicaid Benefits

What Dual Eligible Means in Wyoming

A person is "dual eligible" when they have both Medicare and some form of Medicaid coverage at the same time. In Wyoming, that can mean different things depending on which Medicaid program they're enrolled in.

Partial dual eligible: Enrolled in a Medicare Savings Program (QMB, SLMB, or QI) but not full Wyoming Medicaid. The state pays Medicare premiums and, for QMB, cost-sharing—but doesn't cover services beyond what Medicare covers.

Full dual eligible: Enrolled in both Medicare and full Wyoming Medicaid (ABD Medicaid, with income under $994/month for an individual and assets under $2,000). Full duals get Medicare coverage plus Medicaid benefits including dental, vision, transportation, and potentially home-based care.

Most Wyoming seniors seeking help paying their Part B premium fall into the partial dual category through a Medicare Savings Program.

QMB and Medicare Advantage

One of the most confusing aspects of dual eligibility is how QMB interacts with Medicare Advantage plans. The short answer: QMB works with both Original Medicare and Medicare Advantage, but the billing protections function differently in practice.

Under federal law, QMB beneficiaries cannot be billed for Medicare deductibles, coinsurance, or copayments—regardless of the plan type. With Original Medicare, providers bill the remaining cost-sharing to Wyoming Medicaid through the crossover claims system. Wyoming pays the lesser of the full cost-sharing or the difference between the state Medicaid rate and what Medicare paid.

With Medicare Advantage, the plan itself is responsible for covering cost-sharing. QMB enrollees in a Medicare Advantage plan should have $0 copays for in-network services. But providers—particularly those unfamiliar with QMB rules—sometimes send bills anyway, creating the same billing disputes that Original Medicare QMB enrollees face.

The Special Enrollment Period

Starting January 1, 2025, the Dual/LIS Special Enrollment Period lets your parent make one election each month to switch to Original Medicare and a standalone Part D plan or move between standalone Part D plans. The quarterly SEP ended; it does not let someone switch from one Medicare Advantage plan to another. This flexibility is valuable—it means your parent isn't locked into a plan that doesn't serve them well.

Extra Help Comes Automatically

Every dual eligible—partial or full—automatically qualifies for Extra Help (the Part D Low-Income Subsidy). When WDH approves a Medicare Savings Program, CMS deems the beneficiary eligible for Full Extra Help without a separate application. Generic drug copays drop to roughly $5.10 and brand-name copays to about $12.65, with no Part D deductible.

Combined with the Inflation Reduction Act's $2,100 annual out-of-pocket cap on Part D costs in 2026, dual eligible beneficiaries in Wyoming have substantial protection against prescription drug expenses.

D-SNP Plans in Wyoming

Dual-Eligible Special Needs Plans (D-SNPs) are Medicare Advantage plans designed specifically for people who have both Medicare and Medicaid. These plans often include extra benefits—dental, hearing aids, transportation to medical appointments, over-the-counter drug allowances—tailored to low-income beneficiaries.

D-SNP availability varies by county in Wyoming. Not every area has a D-SNP option, particularly in frontier counties. WSHIIP can check what's available in your parent's zip code.
