West Virginia Medicare Savings Program Renewal: How to Keep Your Benefits

How Renewal Cycles Work

West Virginia doesn't make you reapply from scratch every year, but the Department of Human Services does verify your continued eligibility. The renewal schedule depends on your MSP tier:

QMB and SLMB renew on a 12-month rolling cycle from the month your coverage started. If you were approved in March, your renewal comes around the following March.

QI (QI-1) renews on a calendar-year basis every December, regardless of when coverage began. Because QI is funded through a capped federal grant, the annual December renewal resets the funding cycle. Priority goes to individuals who received QI during the previous year.

Ex Parte Renewal: When the State Does the Work

Under federal regulations, West Virginia must first attempt an ex parte renewal — an automated check using electronic data exchanges with Social Security, state wage databases, and the Asset Verification System. If everything checks out and your income and resources still fall within limits, coverage auto-renews and you receive a notice confirming it.

This is the best-case scenario. No paperwork, no phone calls, no trips to the county office. You get a letter saying your benefits continue.

When You Get a Renewal Packet

If the automated verification doesn't produce enough information — maybe you have income from a source that doesn't report electronically, or your bank account data doesn't match — DoHS mails a redetermination packet. You have 30 days to complete the forms, sign them, and return them with updated financial verifications.

The required documents are the same as the initial application: current bank statements, income documentation (Social Security award letter, pension statements), and any other verification the packet requests.

Miss the 30-day deadline and coverage terminates. But West Virginia offers a safety net: a 90-day reconsideration grace period after termination. If you submit all the requested documents within 90 days of losing coverage, your case can be reopened without filing a brand-new application.

That grace period matters. If you're helping a parent with their renewal and the packet gets buried in a stack of mail, you have three months to recover — but only if you act within that window.

Reporting Changes Between Renewals

You're required to report changes in circumstances to DoHS Bureau for Family Assistance within 10 calendar days. Reportable events include:

  • Changes in gross monthly income (a Social Security COLA increase, a new pension, lost income)
  • Liquid asset changes that push resources above federal limits
  • Relocation to another county or out of state
  • Changes in marital status or household composition

An interstate move requires coordinated case closure with West Virginia DoHS and an immediate application in the new state. MSP eligibility doesn't transfer across state lines — each state runs its own program with its own processing.

A cost-of-living adjustment (COLA) to Social Security is the most common change that affects renewals. If the annual COLA pushes your income above your current tier's ceiling, you may move from QMB to SLMB, or from SLMB to QI. You don't necessarily lose help — you shift to the tier that matches your new income.

What If Your Renewal Is Denied?

If DoHS determines you no longer qualify at renewal and sends a termination notice, you have the right to appeal. File Form IG-BR-29 with the West Virginia Office of Inspector General Board of Review within 90 calendar days of the mail date printed on the notice.

To keep your coverage active during the appeal, you must file within 10 to 13 calendar days of the notice date and request "aid paid pending" status. This continues your MSP benefits while the hearing is processed. If the hearing officer ultimately sides with DoHS, the agency can establish a repayment claim for benefits paid during the appeal period.
