Virginia Medicare Dual Eligible: Benefits, D-SNPs, and QMB
What "Dual Eligible" Means in Virginia
A dual eligible is someone who has both Medicare and some form of Medicaid at the same time. In Virginia, this takes different forms depending on which Medicaid category the person falls into.
Full-benefit dual eligible (FBDE): Has Medicare plus full Virginia Medicaid (the Aged, Blind, or Disabled category). Medicare pays first for Medicare-covered services, and Medicaid fills in the rest — including dental, vision, transportation, and potentially long-term care.
Partial dual eligible: Has Medicare plus a Medicare Savings Program (QMB, SLMB, or QI) only. The MSP pays Medicare premiums and, in QMB's case, cost-sharing. But it doesn't provide full Medicaid services. The person keeps Medicare as their primary coverage and gets financial help with the costs.
Both types are "dual eligible" in the federal classification system, but the practical differences are significant.
D-SNPs vs QMB: Not the Same Thing
Dual-Eligible Special Needs Plans (D-SNPs) are Medicare Advantage plans marketed to people who have both Medicare and Medicaid. They're private insurance products offered by companies like Humana, UnitedHealthcare, and Anthem, with defined provider networks and often extra benefits like dental, vision, hearing, and over-the-counter allowances.
QMB is a state financial subsidy — it pays your Medicare costs but isn't an insurance plan. You don't have to be in a D-SNP to have QMB, and you don't have to have QMB to be in a D-SNP.
Here's the key question families face: should a QMB beneficiary stay on Original Medicare or enroll in a D-SNP?
Original Medicare with QMB gives unrestricted provider choice. Any doctor or hospital that accepts Medicare accepts the patient. QMB handles the cost-sharing. No network restrictions, no prior authorization for Original Medicare services, no referral requirements.
D-SNP enrollment puts the beneficiary in a managed care network. The plan may offer extras (dental, vision, hearing aids, meal delivery), but the tradeoff is provider restrictions, prior authorization requirements, and the complexity of navigating a private plan's rules alongside state Medicaid.
For many QMB beneficiaries — especially those with established doctor relationships or complex medical needs — Original Medicare with QMB provides simpler, broader coverage. D-SNPs make more sense when the extra benefits matter and the provider network includes the beneficiary's current doctors.
How Virginia's Cardinal Care Fits In
Virginia Medicaid operates under the brand name Cardinal Care. When someone is approved for QMB, SLMB, or QI, they receive a Cardinal Care identification card even though their coverage is limited to Medicare cost-sharing assistance.
For full-benefit dual eligibles, Cardinal Care provides the comprehensive Medicaid services beyond what Medicare covers. The Cardinal Care card is what pharmacies, dental offices, and transportation providers use to bill Virginia Medicaid.
The confusing part: both partial and full dual eligibles carry Cardinal Care cards, but the cards unlock different levels of coverage. A QMB-only card gets you Medicare cost-sharing protection. A full Medicaid card gets you the full benefit package.

Medicare Advantage and QMB Together
A QMB beneficiary enrolled in a Medicare Advantage (MA) plan has their MA plan's cost-sharing paid by the state — plan copays, coinsurance, and deductibles are all covered under QMB. But balance-billing protections apply to Medicare-covered services, not plan-specific supplemental benefits.
If you're weighing Medicare Advantage versus Original Medicare for a QMB beneficiary, the Virginia Medicare Savings Programs & Extra Help Guide breaks down how QMB interacts with each option, including what happens to cost-sharing, Extra Help, and provider access under both paths.
