Virginia Extra Help Prescription Assistance for Seniors

What Extra Help Does

Extra Help — formally the Part D Low-Income Subsidy (LIS) — reduces Medicare prescription drug costs to near zero for qualifying beneficiaries. Under the full subsidy, you pay:

  • $0 for the Part D plan premium (up to the regional benchmark amount)
  • $0 annual Part D deductible
  • In 2026, up to $5.10 per generic prescription and $12.65 per brand-name prescription at a participating pharmacy

The 2026 Part D out-of-pocket cap is $2,100 for covered drugs. Extra Help lowers the enrollee's share through statutory copay limits; annual out-of-pocket spending depends on the prescriptions they fill.

How to Get Extra Help in Virginia

There are two paths, and the faster one is through the Medicare Savings Programs:

Path 1: Apply for a Medicare Savings Program at the state level. Anyone approved for QMB, SLMB, or QI in Virginia is automatically deemed eligible for Extra Help. DMAS sends a monthly data file to CMS, and the subsidy activates without a separate application. This is the most reliable route because it eliminates the need to deal with SSA's separate Extra Help application.

Path 2: Apply directly at Social Security. You can file for Extra Help through SSA (ssa.gov/extrahelp or 1-800-772-1213). The standalone Extra Help income limit is 150% FPL — higher than QMB or SLMB — so some people qualify for Extra Help even if their income exceeds MSP limits. But filing at SSA also triggers an automatic MSP application in Virginia, so you may end up in both programs.

Virginia Has No State Pharmacy Assistance Program

Unlike Pennsylvania (PACE) or New Jersey (PAAD), Virginia does not operate a general senior State Pharmaceutical Assistance Program. The Virginia HIV Drug Assistance Program (ADAP) exists but serves only HIV-positive individuals. Other assistance may be available through manufacturer programs or community health center 340B programs, but these are not statewide senior subsidies.

This makes securing Extra Help especially important in Virginia. Without it, there's no state backstop for drug costs.

What Happens After Automatic Enrollment

Once CMS receives the deemed-eligible notification from DMAS:

  • If the beneficiary is already enrolled in a Part D plan, Extra Help kicks in immediately — the plan stops charging the premium (up to benchmark) and applies LIS copay rates at the pharmacy.
  • If the beneficiary has no Part D plan, CMS auto-assigns a benchmark plan. The beneficiary can make coverage changes once per calendar month using the Special Enrollment Period available to people who get Extra Help.

The Pharmacy Lag Problem

Sometimes the pharmacy's billing system doesn't reflect Extra Help status right away. If a pharmacist says the system shows no subsidy, the beneficiary can use CMS's Best Available Evidence (BAE) policy: presenting a copy of the Virginia DSS approval notice or a Medicaid card gives the Part D plan evidence to correct the low-income subsidy status and apply the appropriate cost-sharing at the point of sale.

Keep a copy of the MSP approval letter with medication bottles for exactly this situation.

Extra Help vs. the Medicare Prescription Payment Plan

The Medicare Prescription Payment Plan (M3P) lets beneficiaries spread out-of-pocket drug costs into monthly installments. It does not reduce what you owe — it's a payment plan, not a subsidy. Extra Help actually lowers the amount you pay. Anyone who might qualify for Extra Help should apply for it before relying on M3P.
