Virginia Conditional Part A Enrollment for QMB

Why Virginia Requires an Extra Step

Most people get Medicare Part A for free because they or a spouse paid Medicare taxes for at least 40 quarters (10 years). But if someone doesn't have enough work history — which is common among immigrants, people who worked in non-covered government employment, or spouses who never worked — Part A comes with a monthly premium ($565/month in 2026 for those with fewer than 30 quarters).

QMB can pay that Part A premium. But Virginia is classified as a Part A group-payer state, which means the path to getting QMB when you lack premium-free Part A requires a specific sequence that starts at Social Security, not at your local DSS.

In a Part A buy-in state, you'd simply walk into the Medicaid office, apply for QMB, and the state would handle Part A enrollment automatically. Virginia doesn't work that way.

The Required Sequence

Step 1: Go to a Social Security field office and request conditional Part A enrollment.

This can generally only happen during the General Enrollment Period (January 1 through March 31) or during a qualifying Special Enrollment Period. The SSA application must include a specific remark in the record, per POMS HI 00801.140:

"I wish to enroll in Part A. I understand that I am not eligible for Premium-Free Part A. I choose to enroll conditionally, subject to approval of my QMB application by the State Medicaid agency. I do not want Part A coverage to begin before my State starts paying my monthly premiums."

Without this remark, SSA may process a standard premium Part A enrollment — which would make the applicant personally liable for the premium until QMB is approved.

Step 2: Get the SSA conditional enrollment receipt.

SSA issues documentation confirming the conditional Part A enrollment was processed. Keep this document.

Step 3: Apply for QMB at your local Virginia DSS or through Cover Virginia.

Attach the SSA conditional enrollment receipt to the QMB application. The local DSS evaluates income and resource eligibility as usual.

Step 4: If approved, DMAS adds you to Virginia's Part A Group Payer account.

This signals SSA and CMS to activate Part A coverage. The state pays the Part A premium going forward, and the beneficiary owes nothing.

What Goes Wrong

Applying for QMB first, without the SSA step. If someone goes to their local DSS and applies for QMB before completing conditional Part A enrollment at SSA, the DSS cannot approve QMB. The caseworker should issue a referral directing the applicant to SSA — but this costs weeks and potentially pushes past the General Enrollment Period window.

Missing the enrollment window. The General Enrollment Period runs only January through March. If you miss it, the next opportunity is typically the following January, unless a Special Enrollment Period applies. Under current Medicare rules, standard premium-Part A coverage enrolled in during the GEP starts the month after sign-up, not in July. Conditional Part A in Virginia still requires the state to approve QMB and pay the premium before coverage is activated.

SSA staff unfamiliarity. Conditional Part A enrollment is uncommon enough that some SSA field office staff aren't immediately familiar with the process. Referencing POMS HI 00801.140 by citation can help. Bringing a printed copy of the conditional enrollment language speeds things along.

The Timing Pressure

Because the GEP is only three months long and QMB applications can take up to 45 days to process in Virginia, families in this situation should start early enough to complete the SSA conditional enrollment step, get its receipt, and submit the QMB application to DSS.
