Conditional Part A Medicare Enrollment in Missouri

Why Missouri Is Different

Most people get Medicare Part A for free if they (or a spouse) worked at least 40 quarters paying Medicare taxes. But if you don't have those 40 quarters, Part A comes with a premium — $278/month for 30–39 quarters of work, or $505–$518/month for fewer than 30 quarters.

QMB covers that premium. But enrolling in Part A when you don't have premium-free coverage isn't straightforward in Missouri — because Missouri is classified as a group payer state under SSA's POMS HI 00801.140.

In a "buy-in" state, the state Medicaid agency can directly enroll you in Part A at any time and pay the premium through QMB. In a group payer state like Missouri, FSD cannot enroll you in Part A on its own. You must go through a specific conditional enrollment process at Social Security first.

The Step-by-Step Conditional Process

This process can only happen during the General Enrollment Period (GEP), which runs January 1 through March 31 each year. Miss this window and you wait until next January.

Step 1: Visit your local SSA office during the GEP. Tell the representative you want to file an application for Medicare Part A on a conditional basis.

Step 2: Request conditional processing. You need SSA to process the Part A application without sending you premium bills. The application must include specific mandatory language in the remarks field: that you only want Part A if Missouri approves your QMB application, and that you don't want coverage to begin before the state starts paying your premiums.

Step 3: Get your documentation. SSA should provide a receipt or Form SSA-795 confirming the conditional filing. This document is what FSD needs to move forward on your QMB application.

Step 4: Submit Form IM-1MSP to FSD. Take the SSA conditional receipt to Missouri's Family Support Division and file your QMB application. FSD evaluates your income and resources, and if you qualify, they accrete you to the CMS State Buy-In master file — activating both Part A and Part B coverage simultaneously.

Step 5: If QMB is denied. The conditional Part A application at SSA automatically voids. You are not billed for Part A premiums. The "conditional" flag ensures this safety net.

Why This Matters

Without the conditional filing, you'd face two bad options:

  1. Enroll in Part A directly and pay $278–$518/month in premiums while you wait for QMB approval — with no guarantee of approval
  2. Skip Part A entirely and forgo QMB, which requires Part A enrollment

The conditional route eliminates the financial risk. The premium only activates if QMB is approved and the state starts paying it.

Timing Considerations

QMB coverage is prospective — it starts the first day of the month after FSD approves your application. Part A coverage under conditional enrollment aligns with that same effective date. So the timeline runs: file conditional Part A at SSA during January–March → submit IM-1MSP to FSD → FSD applies the 45-day federal processing standard (administrative delays can extend it) → coverage begins the first of the following month.

If you file the SSA conditional application in February and FSD approves in April, Part A and Part B coverage under QMB would begin May 1.

People Who Already Have Part B

If you already have Part B but not Part A, you can apply for QMB at FSD at any time during the year — you don't need to wait for the GEP. The conditional Part A enrollment process only applies to people who lack Part A entirely. FSD can approve QMB for someone with Part B alone; the Part B premium stops coming out of your Social Security check once the state buy-in takes effect.
