Utah Medicaid Application Timeline: How Long Approval Takes

Between submitting the application and getting an answer from DWS, families are stuck paying nursing home bills out of pocket while wondering if the paperwork they sent was even complete. The official processing timeline has specific deadlines, but the real-world timeline depends almost entirely on whether you submitted everything the first time.

The 45-Day Standard

DWS typically takes 45 days to process a standard long-term care Medicaid application, or up to 90 days if a disability determination is required.

The application date is the business day DWS receives a signed signature page. Not the day you started filling out the myCase portal. Not the day you gathered your first bank statement. The day the signed application hits DWS's system.

If you fax the application to 1-877-313-4717, the date stamp on the fax confirmation is your application date. If you submit through myCase, the system records the upload timestamp.

Why Applications Take Longer

The 45-day estimate applies to the processing period; incomplete applications can take longer when DWS requests additional documentation.

Common triggers for verification requests:

Gaps in the 60-month financial record. A bank account that was open three years ago and closed doesn't show up in current statements. DWS flags the gap and sends a request for the missing period's records.

Both spouses' records not included. Utah counts both spouses' assets and income. Submitting only the applicant's financial history guarantees a request for the spouse's documentation.

Unclear income sources. If bank deposits don't match the income documentation provided (e.g., a recurring deposit that isn't explained by Social Security or pension statements), DWS asks for clarification.

Missing Form 114AR. If an adult child is managing the application but hasn't submitted the authorized representative form, DWS cannot discuss the case with them, creating communication delays.

Each verification request gives you a response deadline. If you miss the deadline stated in the notice, the application can be denied for failure to cooperate, not on the merits.

Form 927: The Clinical Clock

While the financial track runs through DWS, the clinical determination runs separately through DHHS. A licensed healthcare professional conducts the Nursing Facility Level of Care (NFLOC) assessment and completes Form 927 (Waiver Referral / Level of Care Form).

Form 927 has its own clock: it's valid for 60 days from the level-of-care determination date. If the financial determination takes longer than 60 days, the clinical assessment may need to be repeated unless DHHS grants a formal extension. This happens more often than it should, particularly when incomplete financial documentation stretches the DWS timeline past two months.

To avoid this: schedule the clinical assessment close to when you're ready to submit the financial application, not weeks or months in advance. Contact your local Area Agency on Aging to arrange the assessment. The assessor visits the applicant in the facility, evaluates their ADL needs, cognitive status, and medical conditions, and files Form 927 with DHHS.

The Realistic Timeline

For a well-prepared application with complete 60-month records, both spouses' documentation, and a concurrent Form 927 submission, expect:

  • Week 1-2: DWS receives and logs the application, assigns an eligibility worker
  • Week 2-4: Eligibility worker reviews documents, runs the Asset Verification System (AVS) check against financial institutions
  • Week 4-6: If everything checks out, the financial determination is issued. The clinical determination from Form 927 is matched to the financial approval.
  • Total: 4-6 weeks for a clean submission

For an incomplete application requiring one or two rounds of additional documentation:

  • Week 1-2: Initial review, first verification request issued
  • Week 3-4: Family responds, worker reviews supplemental documents
  • Week 4-6: Possible second request, additional review
  • Week 6-10: Determination issued, assuming no further gaps
  • Total: 6-10 weeks, pushing past the Form 927's 60-day validity window

Tracking Progress

Check application status through myCase under "My Benefits." The system shows the current status and any outstanding document requests. If the status hasn't changed after 30 days, call the DWS Eligibility Hotline at 1-866-435-7414.

Ask specifically: "Has an eligibility worker been assigned? Are there any outstanding verification requests? What is the expected determination date?" These questions cut through generic hold messages and get you to actionable information.

What Happens at Approval

Upon approval, DWS issues a notice specifying the coverage start date, the monthly patient liability amount, and the program type (institutional Medicaid, Aging Waiver, or New Choices Waiver). The nursing facility receives separate notice confirming Medicaid coverage and the approved rate.

If your parent's application is denied, the notice includes the specific reason and instructions for requesting a fair hearing. You have 90 days from the notice date to file an appeal with the DWS Division of Adjudications.
