Utah Extra Help for Medicare Part D: Prescription Drug Savings
Your parent picks up a prescription and the pharmacy wants $85 for a generic. They're on Medicare Part D, but the copay is eating through a Social Security check that barely covers rent. In 2026, Extra Help caps the copay at $5.10 for a generic drug.
What Extra Help Pays For
Extra Help — formally the Part D Low-Income Subsidy — reduces Medicare prescription drug costs across the board:
- Part D premium: $0 per month for basic benchmark Part D plans
- Annual deductible: Waived entirely
- Copays: Capped at $5.10 for generic drugs and $12.65 for brand-name drugs in 2026
- Catastrophic coverage: Once out-of-pocket spending hits $2,100, copays drop to $0 for the rest of the year
Social Security estimates Extra Help is worth roughly $5,700 per year in prescription drug savings.
Automatic Enrollment Through MSP
If your parent enrolls in any Utah Medicare Savings Program — QMB, SLMB, or QI — they're automatically deemed eligible for full Extra Help. No separate application to Social Security is needed. CMS flags their record, and the Part D plan applies Extra Help pricing.
This automatic trigger is a significant reason to pursue MSP enrollment even at the QI level. QI only pays the Part B premium ($202.90/month in 2026), but the Extra Help benefit it unlocks can save thousands more on prescriptions.
Utah Has No State Pharmacy Assistance Program
Some states operate their own drug assistance programs (SPAPs) that layer on top of Medicare Part D. Utah does not. There is no state-funded program to fill gaps in Part D coverage or help with non-covered medications.
This makes Extra Help through MSP enrollment the primary route to prescription drug cost relief for low-income Utah seniors. The only disease-specific state program is Utah ADAP for individuals living with HIV, administered through the DHHS Division of Population Health.

Applying for Extra Help Directly
If your parent doesn't qualify for an MSP — income above the QI ceiling of $1,816 (single) — they can apply for Extra Help separately through Social Security. Extra Help eligibility extends to 150% of the federal poverty level with its own resource limits.
Apply at ssa.gov/extrahelp, call Social Security at 1-800-772-1213, or visit a local SSA office.
A useful shortcut works in reverse: if your parent files an Extra Help application with SSA, that data is automatically transmitted to Utah DWS. The state treats it as a formal MSP application with the SSA filing date as the application date. DWS may follow up for additional documentation, but no separate MSP form is needed.
The Pharmacy Delay Problem
After MSP approval, it takes several weeks for CMS to update the national pharmacy benefit databases. If your parent fills a prescription during that window, the pharmacy system may show full retail copays instead of Extra Help rates.
The fix is called Best Available Evidence. Present the DWS approval notice (or a myCase printout showing active MSP status) to the pharmacist. The Part D plan must immediately override the system, apply Extra Help copay rates ($5.10 for generics and $12.65 for brand-name drugs in 2026), and submit documentation to CMS to update the record permanently.
The $2,100 Out-of-Pocket Cap
The Part D redesign under the Inflation Reduction Act established a hard annual cap on beneficiary out-of-pocket prescription spending. Once your parent hits $2,100 in a calendar year, the plan pays 100% of covered drug costs for the remainder of the year. Extra Help enrollees rarely approach this cap because their copays are already so low — but it provides an additional safety net.
The Utah Medicare Savings Programs & Extra Help Guide covers the MSP-to-Extra Help automatic enrollment process, the Best Available Evidence protocol, and how to evaluate Part D plan options alongside Extra Help.
